摘要
目的探讨黄体酮联合坤泰胶囊对围绝经期功能失调性子宫出血患者的疗效以及对子宫平滑肌功能、β-内啡肽(EP)、促性腺激素释放激素(GnRH)、去甲肾上腺素(NE)水平的影响。方法研究选取2015年3月—2018年3月收治的围绝经期功能失调性子宫出血患者96例,将患者随机分为对照组和治疗组,每组均48例。对照组给予黄体酮胶囊。治疗组在对照组治疗基础上给予坤泰胶囊,4粒/次,3次/d,连用3个月经周期。比较两组肾阴虚证症状评分、血清性激素水平、完全止血时间、临床疗效、子宫内膜厚度、子宫平滑肌功能以及EP、GnRH、NE水平。结果治疗结束后,治疗组患者肾阴虚症状腰膝酸软、五心烦热、失眠、盗汗、耳鸣评分以及血清促卵泡生成素(FSH)、黄体生成素(LH)水平明显低于对照组,雌激素(E2)水平显著高于对照组(P<0.01);治疗组总有效率为95.83%,明显高于对照组的78.26%(P<0.05);治疗组患者的完全止血时间明显短于对照组(P<0.01);治疗结束后,治疗组的子宫内膜厚度明显低于对照组,子宫平滑肌收缩幅度、频率,子宫平滑肌功能明显高于对照组(P<0.01);与对照组比较,治疗组患者治疗后体内β-EP水平增加更明显,GnRH和NE下降更明显(P<0.01)。结论黄体酮联合坤泰胶囊治疗围绝经期功能失调性子宫出血患者的疗效明显,且能有效改善患者的子宫平滑肌功能,调节EP、GnRH、NE水平可能与其疗效有关。
Objective To investigate the efficacy of Progesterone combined with Kuntai Capsule in treating peri-menopausal dysfunctional uterine bleeding and its effect on uterine smooth muscle function,levels ofβ-endorphin(β-EP),gonadotropin releasing hormone(GnRH),norepinephrine(NE).Methods Ninety-six perimenopausal dysfunctional uterine bleeding patients admitted to our hospital from March 2015 to March 2018 were selected and randomly divided into control group and treatment group,with 48 cases in each group.The control group was given with Progesterone capsules.On the basis of the treatment of control group,the treatment group was given with Kuntai Capsules with 4 capsules/time and bid for 3 menstrual cycles.Scores of symptoms of syndrome of kidney-Ying deficiency,serum sex hormone level,complete hemostasis time,the efficacy,endometrial thickness,uterus smooth muscle function,and levels of EP,GnRH,NE of were compared between the two groups.Results At the end of the treatment,scores of symptoms of kidney Yin deficiency,such as waist and knee tenderness,fever of five nerves,insomnia,night sweats,tinnitus,serum levels of follicle-stimulating hormone(FSH)and luteinizing hormone(LH)in the treatment group were significantly lower,while estrogen(E2)was significantly higher,than those in the control group(P<0.01).The total effective rate of the treatment group was 95.83%,significantly higher than that of the control group 78.26%(P<0.05);The time of complete hemostasis in the treatment group was significantly shorter than that in the control group(P<0.01).After the treatment,the endometrial thickness of the treatment group was significantly lower,while contractile amplitude and frequency of uterine smooth muscle were significantly higher than those of control group(P<0.01).Compared with the control group,level ofβ-EP of treatment group was evidently increased,and levels of GnRH and NE were remarkably decreased(P<0.01).Conclusion The efficacy of Progesterone combined with Kuntai Capsule in treating peri-menopausal dysfunctional uterine bleeding is significant and can improve the uterine smooth muscle function,and the regulation of EP,GnRH,and NE may be related to its curative effect.
