摘要
目的分析胎盘早剥的产前超声诊断价值及妊娠结局,为提高胎盘早剥诊断率及改善母婴预后提供临床依据。方法选取2016年1月-2018年12月在湖北省妇幼保健院诊断为胎盘早剥的374例产妇,依据产前超声检查结果分为超声阳性组(129例)和超声阴性组(245例)。比较两组产妇的临床表现、妊娠结局、胎盘早剥分度。结果两组产妇阴道出血、胎膜早破、胎心监护异常发生率,新生儿低体质量儿发生率、新生儿重症监护室(NICU)入住率,产妇产后出血、子宫胎盘卒中、死胎发生率,胎盘早剥分度比较,差异均有统计学意义(均P<0.05)。结论严重的胎盘早剥会导致不良妊娠结局,超声检查可提高胎盘早剥的诊断率,改善预后。
Objective To analyze the diagnostic value of prenatal ultrasonography for placental abruption and pregnancy outcomes,provide a clinical basis for improving diagnostic rate of placental abruption and maternal-infantile prognosis.Methods From January 2016 to December 2018,374 postpartum women diagnosed as placental abruption in Hubei Maternal and Child Health Care Hospital were selected and divided into ultrasound positive group(129 cases)and ultrasound negative group(245 cases)according to the results of prenatal ultrasonography.The clinical manifestations,pregnancy outcomes,and degrees of placental abruption in the two groups were compared.Results There were statistically significant differences in the incidence rates of vaginal bleeding,premature rupture of fetal membranes,abnormal fetal heart rate monitoring,low birth weight,neonatal intensive care unit admission rate,postpartum hemorrhage,uteroplacental apoplexy,fetal death,and degrees of placental abruption between the two groups(P<0.05).Conclusion Severe placental abruption can result in adverse pregnancy outcomes.Ultrasonography can improve the diagnostic rate of placental abruption and prognosis.
