摘要
目的探讨超声乳化联合小梁切除术治疗对老年青光眼合并白内障患者的临床疗效及对眼压、视力的影响。方法选取我院2018年6月-2019年6月收治老年青光眼合并白内障的患者82例为研究对象,按手术方法不同分为联合组和对照组,每组41例。联合组给予超声乳化联合小梁切除术,对照组给予小梁切除术。比较2组临床疗效及手术前后眼压、视力情况。结果联合组总有效率显著高于对照组(P<0.05);2组术后1周,术后1、3个月视力均较术前显著提高,且联合组高于对照组(P<0.05)。2组术后1周,术后1、3个月眼压下降,且联合组低于对照组(P<0.05);联合组术后并发症总发生率低于对照组(P<0.05)。结论超声乳化和小梁切除术均可有效治疗老年青光眼合并白内障,二者联合治疗较单独小梁切除术疗效更好,能更有效的恢复患者视力,降低并发症发生率。
Objective To investigate the clinical effect of phacoemulsification combined with trabeculectomy on elderly patients with cataract combined with glaucoma, and their effect on intraocular pressure(IOP) and visual acuity. Methods Eighty-two elderly patients with glaucoma and cataract who were treated in our hospital from June 2018 to June 2019 were selected as the study subjects. According to the operation method, they were divided into combination group and control group, with 41 cases in each group. The combination group received phacoemulsification and trabeculectomy, while the control group received trabeculectomy. The clinical effect, IOP and visual acuity before and after operation were compared between the two groups. Results The total effective rate of the combination group was significantly higher than that of the control group(P<0.05). The visual acuity of the two groups was significantly improved in 1 week, 1 month and 3 months after operation, which was higher in the combination group than in the control group(P<0.05). The IOP in the two groups was reduced in 1 week, 1 month and 3 months after operation, which was lower in the combination group in the control group(P<0.05). The total incidence of postoperative complications in the combination group was lower than that in the control group(P<0.05). Conclusion Both phacoemulsification and trabeculectomy are effective in the treatment of glaucoma with cataract in the elderly patients. However, the combination treatment is better than trabeculectomy alone, which can more effectively restore the vision of patients and reduce the incidence of complications.
