摘要
目的 探讨双切口超声乳化术联合小梁切除术在白内障合并青光眼患者中的治疗效果。方法 选取2016年9月—2020年9月于本院就诊的白内障合并青光眼患者30例,按随机数字表法分为两组。对照组15例采用单切口超声乳化术联合小梁切除术治疗,观察组15例采用双切口超声乳化术联合小梁切除术治疗,两组均观察至术后3个月。对比两组的眼内压、视力水平、并发症及生活质量。结果 术前,两组的眼内压、最佳矫正视力、裸眼视力比较,差异无统计学意义(P>0.05);术后3个月,观察组的眼内压为(13.47±1.85)mmHg,低于对照组,最佳矫正视力、裸眼视力分别为(0.79±0.21)、(0.68±0.14),均高于对照组,组间差异有统计学意义(P<0.05)。观察组的并发症发生率为13.33%,低于对照组的53.33%,差异有统计学意义(P<0.05)。术后3个月,观察组的心理功能、躯体功能、社会功能、物质状态评分分别为(81.69±4.35)分、(76.94±4.11)分、(80.67±4.09)分、(87.25±4.49)分,均高于对照组,组间差异有统计学意义(P<0.05)。结论 采用双切口超声乳化术联合小梁切除术治疗白内障合并青光眼患者的效果理想,可有效降低眼内压,减少术后并发症,促进视力恢复,提升患者生活质量,安全可行。
Objective To investigate the therapeutic effect of double-incision phacoemulsification combined with trabeculectomy in cataract patients with glaucoma.Methods A total of 30 cataract patients with glaucoma treated in our hospital from September 2016 to September 2020 were selected and divided into two groups according to random number table method.15 patients in the control group were treated with single-incision ultrasonic emulsification combined with trabeculectomy,and 15 patients in the observation group were treated with double-incision phacoemulsification combined with trabeculectomy.Both groups were observed until 3 months after surgery.The intraocular pressure,visual acuity,complications and quality of life were compared between the two groups.Results Before surgery,there were no significant differences in intraocular pressure,best corrected visual acuity and naked eye visual acuity between the two groups(P>0.05);3 months after surgery,the intraocular pressure in the observation group was(13.47±1.85)mmHg,lower than that in the control group,and the best corrected visual acuity and naked visual acuity were(0.79±0.21)and(0.68±0.14),respectively,higher than those of the control group,the differences between the groups were statistically significant(P<0.05).The incidence of complications in the observation group was 13.33%,lower than 53.33%in the control group,and the difference was statistically significant(P<0.05).3 months after surgery,the scores of psychological function,physical function,social function and material status in the observation group were(81.69±4.35)points,(76.94±4.11)points,(80.67±4.09)points and(87.25±4.49)points,respectively,which were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Double-incision phacoemulsification combined with trabeculectomy is effective in the treatment of cataract and glaucoma,which can effectively reduce intraocular pressure,reduce postoperative complications,promote visual recovery and improve the quality of life of patients,and is safe and feasible.
