摘要
目的:定量分析并系统评价常规剂量玉屏风散联合抗组胺药用于慢性荨麻疹疗效的影响。方法:计算机联网检索从建库至2019年10月中国知网、中国生物医学文献数据库、维普数据库、万方数据库、PubMed及EMBase等数据库发表的玉屏风散联合抗组胺药治疗慢性荨麻疹的随机对照试验文献(联合组干预措施为常规剂量玉屏风散与抗组胺药联合应用;对照组干预措施为单纯使用常规剂量抗组胺药;抗组胺药包括咪唑斯汀、依巴斯汀、西替利嗪、左西替利嗪、氯雷他定、枸地氯雷他定、非索非那定、奥洛他定、阿伐斯汀、氮卓斯汀、苯磺贝他斯汀及地氯雷他定等)。采用RevMan 5.3和Stata 14.2软件进行荟萃分析(Meta分析)。结果:共纳入28篇文献,涉及1422例患者。Meta分析结果显示,联合组患者的总有效率明显高于对照组(OR=3.85,95%CI=2.91~5.11,P<0.00001),复发率明显低于对照组(OR=0.36,95%CI=0.27~0.49,P<0.00001),差异均有统计学意义;两组患者不良反应发生率比较,差异无统计学意义(OR=0.79,95%CI=0.52~1.20,P=0.27)。结论:相较于单纯使用抗组胺药,玉屏风散联合抗组胺药治疗慢性荨麻疹的疗效更优,复发率更低,且安全性大致相当。
OBJECTIVE:To quantitatively analyze and systematically review the effect of conventional doses of Yupingfeng powder combined with antihistamines on chronic urticaria.METHODS:Randomized controlled trials(the intervening measure of drug combination group was conventional dose of Yupingfeng powder combined with antihistamines;the control group was given conventional dose of antihistamines alone;the antihistamines included mizolastine,ebastine,cetirizine,levocetirizine,loratadine,desloratadine citrate disodium,fexofenadine,olopatadine,acrivastine,azelastine,bepotastine besilate and desloratadine)on Yupingfeng powder combined with antihistamines in the treatment of chronic urticaria published on CNKI,CBM,VIP,Wanfang,PubMed and EMBase from base-building to Oct.2019 were retrieved.RevMan 5.3 and Stata 14.2 were adopted to conduct Meta analysis.RESULTS:A total of 28 studies were involved,including 1422 patients.Results of Meta analysis indicated that the total effective rate of drug combination group was significantly higher than that of the control group(OR=3.85,95%CI=2.91-5.11,P<0.00001),the recurrence rate of drug combination group was significantly lower than that of the control group(OR=0.36,95%CI=0.27-0.49,P<0.00001),with statistically significant differences;there was no statistical significance in difference in incidences of adverse drug reactions between two groups(OR=0.79,95%CI=0.52-1.20,P=0.27).CONCLUSIONS:Compared with antihistamines alone,Yupingfeng powder combined with antihistamines is more effective in the treatment of chronic urticaria,with a lower recurrence rate and roughly equivalent safety.
