摘要
目的观察清热化痰散结方对非哺乳期乳腺炎患者免疫球蛋白G(Ig G)、Ig M及血管内皮生长因子(VEGF)水平的影响。方法将60例非哺乳期乳腺炎患者按照随机数字表法分为2组。对照组30例予一般综合治疗;治疗组30例在对照组治疗基础上加清热化痰散结方治疗。2组均治疗1个月。比较2组疗效;观察2组治疗前后疼痛视觉模拟评分法(VAS)评分、肿块评分、红肿评分变化;比较2组治疗前后炎性腺体组织Ig G、Ig M及VEGF变化。结果治疗组总有效率96.6%(29/30),对照组80.0%(24/30),治疗组疗效优于对照组(P<0.05)。2组治疗后疼痛VAS评分、肿块评分、红肿评分均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。伴脓肿(溃破)、伴窦道者Ig G、Ig M均低于单纯肿块者(P<0.05),伴窦道者Ig G、Ig M均低于伴脓肿(溃破)者(P<0.05);伴脓肿(溃破)、伴窦道者VEGF均高于单纯肿块者(P<0.05),伴窦道者VEGF低于伴脓肿(溃破)者(P<0.05)。2组治疗后炎性腺体组织Ig G、Ig M及VEGF均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。结论清热化痰散结方治疗非哺乳期乳腺炎,能抑制组织的免疫反应,降低VEGF水平,进而缓解症状、体征,提高临床疗效。
Objective To observe the effect of Qingre Huatan Sanjie recipe on the levels of immunoglobulin G(IgG),IgM and vascular endothelial growth factor(VEGF)in inflammatory gland tissne of non lactating mastitis patients.Methods 60 cases of non lactating mastitis were divided into two groups according to the random number table method.30 cases in control group were treated by general comprehensive treatment.30 cases in treatment group were treated by Qingre Huatan Sanjie recipe on the basis of the control group.Both groups were treated for one month.The changes of pain visual analogue score(VAS)score,swelling score and mass score before and after treatment were observed between the two groups.The curative effect of the two groups was compared.The changes of IgG,IgM and VEGF in inflammatory gland tissue were compared before and after treatment between the two groups.Results The total effective rate was 96.6%(29/30)in the treatment group and 80.0%(24/30)in the control group.The clinical curative effect of the treatment group was better than that of the control group(P<0.05).The pain VAS score,swelling score and mass score of the two groups were lower than those of the group before treatment(P<0.05),and the treatment group was significantly lower(P<0.05).The IgM and IgG in patients with abscess(rupture)and sinus were lower than those in patients with simple mass(P<0.05),IgM and IgG in patients with sinus were lower than those in patients with abscess(rupture)(P<0.05).The VEGF in patients with abscess(rupture)and sinus was higher than that in patients with simple mass(P<0.05),and VEGF in patients with sinus was lower than that in patients with abscess(rupture)(P<0.05).After treatment,IgM,IgG and VEGF in the inflammatory gland tissue of the two groups were lower than those before treatment(P<0.05),and the decrease in the treatment group was more significant(P<0.05).Conclusion Qingre Huatan Sanjie recipe can treat non-lactating mastitis,which can suppress the immune response of tissues,reduce the level of VEGF,and then relieve symptoms and signs,and improve clinical efficacy.
