摘要
目的:应用Meta分析系统评价健脾益气清热燥湿法(JPYQQRZS法)治疗慢性糜烂性胃炎(CEG)的疗效。方法:计算机系统检索The Cochrane Library、Pub Med、中国知网、万方、CBM等数据库(检索时间范围为2000年1月至2018年1月),以JPYQQRZS法治疗CEG与常规西药治疗本病的随机对照试验(RCT)为研究对象,采用改良JADAD量表记分法进行质量评估并以REV5.3软件进行Meta分析。结果:共纳入19项研究3379例CEG患者,对纳入文献进行质异性检验得P=0.39,I^2=5%,表示数据具有同质性,当采用固定效应模型。此外OR=3.61.95%CI:2.74~4.75,合并效应的检验Z=9.12,P<0.00001,证明JPYQQRZS法与常规西药治疗的总体有效率差异有统计学意义。结论:JPYQQRZS法治疗慢性糜烂性胃炎在整体疗效、胃镜下表现及胃脘痛、痞满等症状缓解上优于常规西药治疗,但在HP根除上不存在优势,仍需扩大样本数量纳入更多高质量RCT进行分析;黄连与黄芪药对兼顾寒热、升降、虚实,为JPYQQRZS法首推的药对组合。
To evaluate the therapeutic effect of invigorating spleen and tonifying Qi and clearing heat and dampness(JPYQQRZS) on chronic erosive gastritis(CEG) by Meta analysis system. Methods: Computer system was used to search the databases of The Cochrane Library,Pub Med, Zhiwang, Wanfang, etc.(the search time ranged from January 2000 to January 2018). The randomized controlled trial(RCT) was used to study the treatment of CEG with JPYQQRZS and routine western medicine. Use modified JADAD scale scoring method to evaluate quality and use Rev 5.3 the software was analyzed by Meta. Results: A total of 3379 patients with CEG were included in 19 studies. The qualitative heterogeneity test was performed on the literature. The results showed that the data had homogeneity, and the fixed effect model should be used. In addition, the test of the combined effect of OR=3.61.95%CI 2.74~4.75, showed that the total effective rate of JPYQQRZS was significantly different from that of conventional western medicine. Conclusion:JPYQQRZS method in the treatment of chronic erosive gastritis in the overall efficacy, gastroscopy The symptoms of epigastric pain and fullness were better than that of western medicine. However, there is no advantage in HP eradication, and it is still necessary to expand the sample size to include more high-quality RCT for analysis. Rhizoma Coptidis and Radix Astragali were the combination of cold and heat, rising and falling, deficiency and deficiency, which were the first drug pairs of JPYQQRZS method.
