摘要
胃癌前状态性疾病(活动期)是指消化性溃疡和糜烂性胃炎等疾病,其临床表现以胃脘灼痛、吞酸嘈杂、口干口苦、舌红苔黄,或腻或腐、脉弦为主要证候表现,在胃镜下可见局部红肿、糜烂、溃疡等,其病机是以脾虚为本,毒热为标。现代医学认为其与幽门螺旋杆菌、胃酸过多等因素有关。半夏泻心汤来源与《伤寒杂病论》,其有清热解毒、燥湿降逆,和中消痞之功,体现了辛开苦降、寒温并用、攻补兼施、升降有序之配伍特点。胃癌前状态性疾病(活动期)与半夏泻心汤证病机吻合,基于此,该文从半夏泻心汤的来源与特点,历代医家对其病因病机的认识,及临床应用半夏泻心汤应抓住寒、热、虚、实的病机及证候特点,从中医学药性理论分析其功用,并且总结了诸位医家认为半夏泻心汤君药是"半夏"还是"黄连"的不同认识,提出了本人对半夏泻心汤君药为"黄连"的观点,其能清热燥湿解毒,抗菌制酸,指出其是治疗本病的关键药物,进而揭示了半夏泻心汤治疗胃癌前状态性疾病的机理。
Gastric precancerous state disease(active stage)refers to peptic ulcer,erosive gastritis and other diseases.Its clinical manifestations are burning pain in the stomach,noisy acid swallowing,dry and bitter mouth,red tongue and yellow fur,or greasy or rotten,and string-like pulse.Under gastroscope,local swelling,erosion,ulcer and so on can be seen.Its pathogenesis is based on spleen deficiency and marked by toxic heat.Modern medicine believes that it is related to factors such as Helicobacter pylori and excessive gastric acid.Banxia Xiexin Decoction comes from Treatise on Febrile and Miscellaneous Diseases,which has the functions of clearing away heat,detoxifying,drying dampness and mediating Qi of middle-Jiao and relieving the distension,embodying the characteristics of pungent-opening and bitter-descending method,cold and warm in combination,tonification and purgation in combination,ascending and descending orderly.The pathogenesis of precancerous state disease(active stage)of gastric cancer is consistent with that of Banxia Xiexin Decoction.Based on this,this paper analyzed the function of Banxia Xiexin Decoction from the source and characteristics,the understanding of the etiology and pathogenesis,and the clinical application,which should grasp the pathogenesis and syndrome characteristics of cold,heat,deficiency and excess,and analyzed its function from the theory of traditional Chinese medicine property,and summarized the opinions of various physicians which monarch drug in the prescription was the different cognition of"Banxia(Pinelliaternata)"or"Huanglian(Coptis)".It put forward the author’s view that the monarch medicine of Banxia Xiexin Decoction is"Huanglian(Coptis)".It can clear away heat,dry dampness,detoxify,kill bacteria and inhibit gastric acid.It is the key drug to the treatment of this disease.Furthermore,it reveals the mechanism of Banxia Xiexin Decoction in the treatment of gastric precancerous state diseases.
