摘要
目的探讨妊娠期高血压疾病孕妇生化指标及母儿并发症。方法选取2010年5月~2016年5月的孕产妇共16375例,其中1538例孕产妇患有妊娠期高血压疾病,依照我国妊娠期高血压疾病分类标准,将其中492例患者分为三组,分别为重度子痫前期组与轻度子痫前期组以及妊娠期高血压组,观察三组患者的部分生化指标和母儿并发症情况。结果妊娠期高血压疾病的患病率约为5.58%,其中主要为初产妇,占78.07%,重度子痫前期患者的血清总蛋白(TP)、血浆白蛋白(ALB)低于轻度子痫前期组与妊娠期高血压组,且轻度子痫前期组低于妊娠期高血压组;和其他两组比较,重度子痫前期组的胎盘早剥等严重并发症的发生率显著增加(P<0.05)。结论重度子痫前期的孕产妇和围产儿均有较多的并发症,这是导致死亡的主要原因,且随着患病程度加重,TP与ALB进一步降低,新生儿的并发症发生率也会随之增加。
Objective To explore biochemical indexes and maternal and infant complications in pregnant women with gestational hypertension. Methods 16 375 pregnant women from May 2010 to May 2016 were selected. Among whom, 1538 pregnant women were with gestational hypertension. According to the classification standards of gestational hypertension in China, 492 patients were divided into three groups such as severe preeclampsia group, mild preeclampsia group and gestational hypertension group. Partial biochemical indexes and maternal and infant complications were observed in the three groups. Results The prevalence rate of gestational hypertension was about 5.58%. Among whom, primipara were the most, accounting for 78.07%. Serum total protein(TP) and plasma albumin(Albumin, ALB) in patients with severe preeclampsia were lower than those in mild preeclampsia group and gestational hypertension group. And those of the mild preeclampsia group were lower than the gestational hypertension group. Compared with the other two groups, the incidence of serious complications such as placental abruption in the severe preeclampsia group was significantly increased(P < 0.05). Conclusion Both pregnant women and perinatal children with early severe preeclampsia have more complications, which is the leading cause of death. As the severity of the disease worsens, TP and ALB decrease further and the incidence of complications in newborns increases.
