摘要
目的探讨胎盘绒毛膜血管瘤的临床病理特点、诊断和鉴别诊断。方法对自2015年10月至2017年9月经病理学检查确诊的85例胎盘绒毛膜血管瘤患者的临床及病理资料进行回顾性分析。结果①85例胎盘绒毛膜血管瘤中,单胎51例(60.00%),双胎34例(40.00%)。其中妊娠期经超声发现血管瘤3例(3.53%);胎盘标本大体检查发现7例(8.24%),其中6例(7.06%)为单发血管瘤,1例(1.18%)为多发血管瘤。大体检查未见者78例(91.76%)。②85例母儿并发症或合并症中,脐带扭转29例(34.12%),重度子痫前期21例(24.71%),脐带缠绕19例(22.35%),妊娠期糖尿病11例(12.94%),子痫前期8例(9.41%),妊娠合并甲状腺功能减退6例(7.06%)。③母儿结局中,胎儿畸形引产者6例(7.06%),胎儿生长受限2例(2.35%),新生儿左肾缺如1例(1.18%)等。结论胎盘绒毛膜血管瘤常伴发多种妊娠并发症或合并症,早期诊断和定期随访,可能有助于母儿结局的改善。
Objective To investigate the clinical and pathological features,diagnosis and differential diagnosis of placental chorioangioma.Methods The clinical and pathological data of 85 cases of placental chorioangioma diagnosed by pathology from October 2015 to September 2017 were retrospectively analyzed.Results①Among 85 cases of placental chorioangioma,51 cases(60.00%)were single fetus,34 cases(40.00%)were twins.Among them,3 cases(3.53%)were found by ultrasound.7 cases were found by the naked eye(8.24%),of which 6 cases(7.06%)were single and 1 case(1.18%)was multiple.78 cases(91.76%)were not seen.②Maternal and child complications:umbilical cord torsion in 29 cases(34.12%),severe preeclampsia in 21 cases(24.71%),and umbilical cord entanglement in 19 cases(22.35%).Followed by 11 cases(12.94%)of gestational diabetes mellitus,8 cases(9.41%)of preeclampsia,and6 cases(7.06%)of pregnancy with hypothyroidism.③Maternal and child outcomes:6 fetal malformation cases were induced labor(7.06%).Fetal growth restriction in 2 cases(2.35%),and neonatal left kidney deficiency in one case(1.18%),etc.Conclusion Placental chorioangioma is often accompanied by multiple pregnancy complications.Early diagnosis and regular follow-up may help to the improve maternal and infant outcomes.
