摘要
目的探讨老年人生活自理能力衰减拐点,为社区医务工作者提供精准养老服务提供参考依据。方法采用方便抽样,选取福建省闽东、西、中、南、北地区年龄大于60岁的575名老年人为研究对象。采用日常生活自理能力量表(activity of daily living,ADL)进行筛选,根据评分结果将老年人分为ADL功能完好组(n=389)和受损组(n=175)。绘制年龄预测受试者工作特征曲线(receiver operating characteristic curve,ROC曲线)以确定最佳截断值,并进行Logistic回归分析,联合ROC曲线下面积(area under curve,AUC)评价年龄诊断ADL衰减的预测价值。结果老年人ADL功能完好组的平均年龄(70.59±5.93)岁低于功能受损组(78.81±5.24)岁,年龄与ADL得分具有强相关性(r=-0.65,P<0.01);年龄预测ADL衰减的最佳截断值为75.5岁,ROC曲线线下面积AUC为0.851,最大约登指数为0.612,诊断敏感度为77.1%,特异性为84.1%;年龄大于75岁是ADL的独立危险因素。结论老年人ADL衰减拐点约为75.5岁,其预测准确性良好且具有较好的灵敏度和特异性,能为社区养老决策者考虑进行分级养老服务的方向提供参考价值。
Objective To explore the attenuation inflection point of the elderly’s self-care ability,and to provide reference for the community medical workers to provide accurate old-age services.Methods By convenience sampling method,575 elderly people aged over 60 years from the eastern,western,central,southern and northern regions of Fujian Province were investigated.The Katz activity of daily living(ADL)scale was used for screening.According to the scores,the elderly patients were divided into ADL functional group(n=389)and functional impairment group(n=175).The ROC curve of age prediction ADL attenuation was plotted to determine the optimal cutoff value.Logistic regression analysis was performed to evaluate the predictive value of age-diagnosed ADL attenuation in combination with ROC curve.Results The mean age of the ADL functional group was 70.59±5.93,which was lower than that of the functionally impaired group(78.81±5.24),and the age had a strong correlation with the ADL score(r=-0.65,P<0.01).The best cut-off value for age prediction of ADL attenuation was 75.5 years old,and the area under the ROC curve line AUC was 0.851,the most approximate index was 0.612,the diagnostic sensitivity was 77.1%,and the specificity was 84.1%.Age greater than 75 years was an independent risk factor for ADL.Conclusions The ADL attenuation inflection point of the elderly is about 75 years old.The prediction accuracy is good and has good sensitivity and specificity.It can provide reference value for the community pension decision-makers to consider the direction of grading pension service.
