摘要
目的探讨超声引导下经皮经肝胆囊穿刺造瘘术(PTGCD)在老年性急性胆囊炎治疗中的应用价值。方法回顾性分析贺州市人民医院2014年6月至2017年12月期间收治的85例老年急性胆囊炎患者的临床诊治资料,根据治疗方式的不同分为术前造瘘术后再行择期手术组45例(术前造瘘组)和直接外科手术组40例,比较两组患者的治疗效果及术后并发症。结果术前造瘘术组患者的胆囊炎症得到控制时间为(1.14±0.28)d,与直接外科手术组的(1.27±0.32)d比较差异无统计学意义(P>0.05);术前造瘘组患者术后并发症发生率为2.22%(1/45),明显低于直接外科手术组的15.00%(6/40),差异有统计学意义(P<0.05)。结论超声引导下经皮经肝胆囊穿刺造瘘术的治疗可以迅速缓解老年急性胆囊炎引起的严重症状,疗效可靠,为老年急性胆囊炎二期择期手术提供重要的临床价值。
Objective To explore the value of ultrasound-guided percutaneous transhepatic gallbladder puncture(UG-PTGD) in the treatment of senile acute cholecystitis. Methods The clinical diagnosis and treatment data of 85 elderly patients with acute cholecystitis, who admitted to Hezhou People’s Hospital from June 2014 to December 2017, was retrospectively analyzed. According to different treatment methods, the patients were divided into the preoperative ostomy and elective surgery group(n=45, preoperative sputum group) and direct surgery group(n=40). The clinical effect and postoperative complications of the two groups were compared. Results The control time of gallbladder inflammation in the preoperative ostomy group was(1.14±0.28) d versus(1.27±0.32) d in the direct surgery group(P>0.05);The postoperative complication rate of the preoperative sputum group was 2.22%(1/45), which was significantly lower than 15.00%(6/40) of the direct surgery group(P<0.05). Conclusion Ultrasound-guided percutaneous transhepatic gallbladder puncture and ostomy can quickly alleviate the severe symptoms caused by acute cholecystitis in the elderly,with reliable efficacy, and provide important clinical value for elective surgery for elderly patients with acute cholecystitis.
