摘要
目的:探讨布地奈德联合沙丁胺醇雾化吸入治疗对支气管哮喘急性发作患者血清高迁移率族蛋白B1(HMGB1)水平的影响。方法:选取2017年1月—12月在本院接受治疗的600例支气管哮喘急性发作的患者作为研究对象,按照随机性原则将其随机分为对照组300例和联合组300例。对照组给予常规治疗,包括抗感染、吸氧、平喘、祛痰、镇咳,联合组患者在上述治疗的基础上联合给予布地奈德与沙丁胺醇雾化吸入治疗,布地奈德混悬液1 mg+沙丁胺醇1ml+2. 5 ml氯化钠注射液,应用PARI压力雾化泵通过面罩或口含器雾化吸入,每次应用7~10 min,直至药物完全雾化完,3次/d。7 d为1疗程,治疗1~2个疗程。比较两组患者的临床治疗效果及治疗前后肺功能和血清HMGB1水平变化情况。结果:联合组治疗总有效率(治愈+显效+有效)为93. 67%,显著高于对照组的78. 67%(P <0. 05);治疗后两组患者的用力呼吸肺活量(FVC)、峰值呼气流速(PEF)及第一秒用力呼吸容积(FEV1)较治疗前均显著升高,且联合组患者治疗后上述肺功能指标均显著高于对照组(P <0. 05);治疗前两组患者血清HMGB1水平比较差异无统计学意义(P>0. 05),治疗后较治疗前均显著降低,且联合组治疗后血清HMGB1水平显著低于对照组(P <0. 05)。结论:支气管哮喘急性发作患者在常规治疗的基础上联合布地奈德与沙丁胺醇雾化吸入可有效缓解临床症状,改善肺功能指标;此外,其还可有效促进血清HMGB1水平的降低,从而可有效改善患者肌体炎症状态,抑制病情进一步发展。
Objective:To investigate the effect of budesonide combined with salbutamol aerosol inhalation on serum high mobility group protein 1(HMGB1)levels in patients with acute attack of bronchial asthma.Methods:600 patients with acute attack of bronchial asthma were randomly divided into control group(300 cases)and combined group(300 cases).Both groups were given routine treatment,the combined group of patients were given budesonide and salbutamol aerosol inhalation therapy on the basis of the above treatment.Budesonide suspension 1 mg+salbutamol 1 ml+2.5 ml saline was inhaled by PARI pressure atomization pump through mask or mouth holder for 7-10 minutes each time until the drug was completely atomized,three times a day.7 days was a course of treatment,1-2 courses of treatment.The clinical effects and the changes of lung function and serum HMGB1 level before and after treatment were compared.Results:The total effective rate(cure+marked effect+effective)was 93.67%,which was significantly higher than 78.67%of the control group(P<0.05).After treatment,the above indexes of lung function were significantly higher than those of the control group(P<0.05);after treatment,the levels of HMGB1 in the combined group were significantly lower than those before treatment(P<0.05).Conclusion:On the basis of routine treatment,inhalation of budesonide and salbutamol can effectively alleviate clinical symptoms and improve pulmonary function.In addition,inhalation of budesonide and salbutamol can also effectively promote the decrease of serum HMGB1 level,which can effectively improve the inflammatory state of patients and inhibit the further development of the disease.
