摘要
目的了解上海市崇明区影响低月龄小儿麻疹发病因素,为制定预防措施提供依据;方法收集2015年5月至2016年4月在上海市新华医院崇明分院妇科及儿科就诊的孕妇及小儿静脉血清,采用ELISA方法检测麻疹Ig G抗体水平,分析母婴抗体水平的相关性;结果新生儿出生时胎传抗体水平阳性率、保护率分别为86.95%和47.82%,随着月龄的增长3~5月龄锐减,6~7月龄时基本消失;8~12月龄时由于初免接种抗体水平提高到到90.00%和72.86%,但8月龄及时接种率只有65.71%。胎传抗体与母体的抗体水平有关,目前18~41岁孕妇抗体阳性率、保护率分别为88.78%和47.67%,免疫史不详达79.72%。结论胎传抗体消失及初免疫苗的不及时接种是1岁以下小儿麻疹发病的根本病因,建议以完善网络疫苗接种信息系统为契机,充分开展对育龄女性开展麻疹强化免疫工作,以提高胎传抗体保护水平,同时提高及时接种率。
Objective To understand the influence factors of measles in children under 1 year old in Chongming Area,Shanghai,and provide basis for the development of preventive measures.Methods The serum of pregnant women and children under 1 year old was collected in the Departments of Obstetrics and Gynecology of Shanghai Xinhua Hospital Chongming Branch during May 2015-April 2016,the measles IgG antibody was detected by ELISA method;the correlation of IgG antibody between mother and child was analyzed.Results The positive and protective of fetal antibody in newborns were 86.95%and 47.82%respectively at birth,With the growth of age,it dropped in 3-5 months old infant,basically disappeared in 6-7 month old infant,increased in 8-12 months old infant due to the initial vaccination,however,the timely vaccination rate in 8-month old infant was only 65.71%.The fetal antibody was associated with the antibody level of the mother,The current positive rate and protection rate of the 18-41 year old pregnant women were 88.78%and 47.67%,and 79.72%of them with unknown immunization history.Conclusion The disappearance of fetal transmission antibodies and the lack of timely vaccination was the underlying cause of measles infection in children under 1 year old in Chongming,It is recommended to carry out measles immunization work for women of childbearing age by applying the network vaccination information system,and,at the same time,to improve the timely vaccination rate.
