摘要
目的分析伽玛刀或全脑放射疗法联合吉非替尼对肺癌脑转移瘤患者生活质量、生存率和毒性反应的影响。方法选取本院84例肺癌脑转移瘤患者,分为对照组给予全脑放射疗法与吉非替尼联合治疗,研究组给予伽玛刀与吉非替尼联合治疗,两组各42例,对比两组生活质量、生存率和毒性反应的情况。结果研究组总有效率(92.86%)与对照组对比(85.71%),并无明显差异(χ~2=1.09,P>0.05);研究组Karofsky得分有效率(76.19%)较对照组高(52.38%)(χ~2=5.79,P<0.05);两组治疗后1年、2年生存率对比均无明显差异(χ~2=0.49、0.36,P均>0.05);研究组毒性反应发生率较对照组低(P<0.05)。结论伽玛刀联合吉非替尼能够有效控制肺癌脑转移瘤患者的肿瘤进展,进而减轻毒性反应,有助于提高患者生存率,改善生活质量。
Objective To analyze the effect of gamma knife or whole brain radiotherapy combined with gefitinib on the quality of life,survival rate and toxicity in patients with lung cancer with brain metastases.Methods 84 lung cancer patients with brain metastases were selected in our hospital and divided into the control group receiving the whole brain radiotherapy combined with gefitinib and the research group undergoing gamma knife combined with gefitinib,with 42 cases in each group.The quality of life,survival rate and toxicity were compared between the two groups.Results The total efficiency in the study group(92.86%)showed no significant difference with the control group(85.71%)(χ2=1.09,P>0.05).The efficiency rate of Karofsky scores in the study group(76.19%)was higher than that in the control group(52.38%)(χ2=5.79,P<0.05).The survival rate of 1 year and 2 years after treatment showed no significant difference between the two groups(χ2=0.49,0.36,P both>0.05).The occurrence of toxicity in the study group was lower than that in the control group(P<0.05).Conclusion Gamma knife combined with gefitinib can effectively control the tumor progression of patients with lung cancer with brain metastases,which can reduce the toxicity rate and increase the survival rate,so it can improve their quality of life.
