替莫唑胺联合SRS对脑转移瘤患者血清MBP及NSE的影响

Effects of temozolomide combined with SRS on serum level MBP and NSE in patients with metastatic tumor of brain

摘要 目的探讨替莫唑胺联合头部伽玛刀治疗(SRS)对脑转移瘤患者血清髓鞘碱性蛋白(MBP)及神经元特异性烯醇化酶(NSE)的影响。方法选取2017年9月~2019年12月于我院肿瘤科治疗80例脑转移瘤患者为研究对象,根据随机数字表法分为常规组和联合治疗组,每组40例。常规组给予立体定向伽玛刀放射治疗,联合治疗组在常规组的基础上联合替莫唑胺治疗。观察并比较两组患者的临床疗效、血清中MBP、NSE的水平变化,统计治疗期间发生的不良反应。结果联合治疗组总有效率65.0%(26/40),明显高于常规组的40.0%(16/40),两组间比较,差异有统计学意义(P<0.05)。进行治疗前,两组患者血清中MBP、NSE水平无显著性差异(P>0.05);治疗后,两组患者血清MBP及NSE均显著下降(P<0.05),且联合治疗组血清MBP及NSE显著低于对照组(P<0.05)。两组患者出现的不良反应如白细胞减少、贫血、血小板减少等,进行对比后均无统计学意义(P>0.05)。结论替莫唑胺联合头部伽玛刀治疗相比单纯放射治疗而言,能改善脑转移瘤患者脑组织损伤,降低血清MBP及NSE阳性表达水平,且安全性较好。 Objective To explore the effects of temazolomide combined with gamma knife of stereotactic radiosurgery(SRS)on serum level of myelin basic protein(MBP)and neuronspecific enolase(NSE)in patients with metastatic tumor of brain.Methods 80 patients with metastatic tumor of brain who were treated in our hospital from September 2017 to December 2019 were selected as the study subjects,all of them were randomly divided into the conventional group and the combined treatment group,40 cases in each group.The conventional group was given the stereotactic radiosurgery,while the combination group was treated with temozolomide on the basis of the conventional group.The clinical efficacy and serum MBP and NSE levels in the two groups were observed and compared,and the adverse reactions occurred during the treatment were counted.Results The total effective rate of the combined treatment group was 65.0%(26/40),significantly higher than that of the conventional group,which was 40.0%(16/40).The difference between the two groups was statistically significant(P<0.05).Before treatment,there was no significant difference in serum MBP and NSE levels between the two groups(P>,0.05).After treatment,serum MBP and NSE in both groups decreased significantly(P<0.05),and serum MBP and NSE in the combined treatment group were significantly lower than those in the conventional group(P<0.05).Adverse reactions such as leukopenia,anemia and thrombocytopenia in the two groups were not statistically significant after comparison(P>0.05).Conclusion Temozolomide combined with gamma knife of stereotactic radiotherapy can improve brain tissue damage in patients with metastatic tumor of brain,reduce serum MBP and NSE positive expression levels,and the safety is better.
