摘要
目的分析308准分子光联合点阵激光综合治疗白癜风的临床疗效及患者满意度。方法选择2014年7月—2016年7月我院收治70例白癜风患者,根据诊治时间分为对照组(n=35例,皮损211处)和联合组(n=35例,皮损217处);对照组给予点阵激光治疗和复方倍他米松注射液;联合组在对照组治疗的基础上给予308 nm准分子光治疗。比较两组的临床疗效,治疗后不良反应及患者满意度、血清抗酪氨酸酶抗体水平变化。结果联合组治疗后临床总有效率为94.93%,明显高于对照组(83.41%,P<0.05)。治疗期间,联合组有21处皮损(9.68%,12/217)出现不同程度的红斑或水疱,对照组有10处皮损(4.74%)出现水肿、红斑或水疱等不良反应,2组相比差异无统计学意义(χ~2=1.802,P>0.05)。与治疗前相比,治疗后1个月、3个月2组抗酪氨酸酶抗体水平均明显下降,且联合组显著低于对照组;联合组治疗后患者总满意度为88.57%,明显高于对照组的77.14%(P<0.05)。结论 308 nm准分子光联合点阵激光综合治疗白癜风疗效显著,无严重不良反应,可有效降低抗酪氨酸酶抗体水平,提高患者满意度。
Objective In order to analyze the clinical efficacy and satisfaction of 308 nm excimer laser combined with laser comprehensive treatment on vitiligo. Methods From July 2014 to July 2016,seventy cases of vitiligo patients treated in our hospital,were divided into control group （n=35 cases,211 lesions） and combination group （n=35 cases,217 lesions）,according to the time of diagnosis and treatment. Control group were treated by laser and compound betamethasone injection. Combination group were given 308 nm excimer,based on the treatment in control group. The clinical efficacy,the level of serum anti tyrosinase antibody,adverse reactions and satisfaction were compared between the two groups. Results The total effective rate of combined treatment group was 94.93%,and it was significantly higher than the control group （83.41%,P〈0.05）. During the treatment,the combined group had 21 lesions （9.68%,12/217） with varying degrees of erythema or blisters,and the control group had 10 lesions （4.74%） appeared edema,erythema and blisters and other adverse reactions. There was no significant difference between two groups （χ2=1.802,P〉0.05）. Compared with before treatment,the level of anti tyrosinase antibody in two groups were significantly decreased,and the level of anti-tyrosinase antibody in the combination group was significantly lower than that in the control group after treatment for one month and three months. The overall satisfaction rate of patients in combination group was 88.57%,and it was significantly higher than the control group 77.14% （P〈0.05）. Conclusion 308 nm excimer laser combined with laser has significant effects on vitiligo,and without serious adverse reactions,which can effectively reduce the level of anti tyrosinase antibody and improve patient satisfaction.
