多奈哌齐联合经颅磁刺激对脑卒中患者认知障碍的疗效观察

Effect of Donepezil Combined with Transcranial Magnetic Stimulation on Cognitive Impairment in Stroke Patients

摘要 目的探讨多奈哌齐联合经颅磁刺激在脑卒中认知功能障碍(PSCI)患者中的治疗效果.方法选取2019年5月—2021年5月我院收治的84例PSCI患者为研究对象,按随机数字表法分为两组,每组42例.对照组采用多奈哌齐治疗,观察组在此基础上采用经颅磁刺激治疗,均持续治疗4周.对比两组的治疗效果、认知功能、神经功能及日常生活能力.结果观察组的治疗总有效率为95.24%,高于对照组的80.95%,差异有统计学意义(P<0.05).治疗后,观察组的蒙特利尔认知功能评估量表评分、P300波幅水平、日常生活活动能力量表评分分别为(26.63±1.42)分、(7.22±0.85)V、(82.24±5.33)分,均高于对照组,观察组的美国国立卫生研究院卒中量表评分为(6.88±1.10)分,低于对照组,观察组的P300潜伏期为(334.24±31.68)ms,短于对照组,组间差异有统计学意义(P<0.05).结论多奈哌齐联合经颅磁刺激能够增强PSCI的治疗效果,减轻患者认知功能损伤,提高其日常生活能力. Objective To investigate the therapeutic effect of donepezil combined with transcranial magnetic stimulation(TMS)in patients with post-stroke cognitive impairment(PSCI).Methods A total of 84 PSCI patients admitted to our hospital from May 2019 to May 2021 were selected as the study subjects,and were divided into two groups according to random number table method,with 42 patients in each group.The control group was treated with donepezil,and on this basis,the observation group was additionally treated with TMS.Both groups continued treament for 4 weeks.The therapeutic effect,cognitive function and activity ability of the two groups were compared.Results The total effective rate of the observation group was 95.24%,higher than 80.95%of the control group,the difference was statistically significant(P<0.05).After treatment,the Montreal cognitive assessment scale score,P300 amplitude and activity of daily living scale score of the observation group were(26.63±1.42)points,(7.22±0.85)V and(82.24±5.33)points,respectively,which were higher than those of the control group,the National Institute of Health stroke scale score of the observation group was(6.88±1.10)points,which was lower than that of the control group,the P300 latency of the observation group was(334.24±31.68)ms,which was shorter than that of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Donepezil combined with transcranial magnetic stimulation can enhance the therapeutic effect of PSCI,reduce the cognitive impairment of patients,and improve their ability of daily living.
