摘要
目的分析处方前置审核系统对提高我院门诊合理用药水平的影响。方法介绍根据《处方管理办法》、《医院处方点评管理规范(试行)》等审方规则和我院工作建立的处方前置审核系统在处方审核中的应用。收集我院使用该系统后,2017年6-8月门诊处方审核及干预等情况,并评价其应用效果。结果我院2017年6-8月系统预审核处方667 118张/次,系统预审有问题处方占11.03%,医生主动返回修改处方占比7.57%,药师审核处方占比3.46%,药师复审合格处方占比呈上升趋势,而药师审核处方、药师干预处方及干预后医生返回修改处方占比呈现下降趋势,门诊处方合格率均在97.58%以上。其中消化科门诊和肝脏外科门诊的处方合格率变化明显,分别提高了7.95%和9.96%,8月处方合格率均在95.65%以上。结论处方前置审核能有效规范医生的开方行为,提高合理用药水平和处方质量,促进安全合理用药。
Objective To analyze the effect of prescription pre-checking system on improvement of rational drug use in outpatient of our hospital. Methods The prescription pre-checking system was established based on review of prescription rules ＂Management Policy＂ and＂ Regulations for Hospital Prescription Evaluation （Trial）＂ and hospi- tal-related work. The data of outpatient prescription checking and intervention from June to August in 2017 were collect- ed in our hospital after the application of system,and the effect of the system was evaluated. Results The system pre- checked 667 118 sheets/time of prescriptions from June to August in 2017, of which 11.03 % had problems ： the propor- tion of prescriptions doctors initiatively returned for modifications was 7.57% ,and 3.46% prescriptions were checked by pharmacists. An upward trend appeared in the proportion of prescriptions rechecked by pharmacists, while a down- ward trend presented in the proportion of prescriptions pre-checked or intervened by pharmacists and returned by doc- tors. The qualification rate of outpatient prescription was over 97. 58%. The qualification rate of prescription of outpa- tient of digestive department and liver surgery department was increased by 7. 95% and 9. 96%. The prescription quali- fication rate was above 95.65% in August. Conclusion The prescription we-checking can effectively standardize the doctors＇ behavior of prescription, improve the quality of outpatient prescriptions, and promote safe and rational drug use.
