摘要
目的：探讨益气养阴通络法治疗气阴两虚型IgA肾病的临床效果。方法：选取医院肾内科收治的气阴两虚型IgA肾病患者164例,按随机数字表法分组,对照组82例予以西医常规治疗,研究组82例在对照组基础上予以益气养阴通络汤治疗,检查记录两组间肾功能指标、尿液指标、外周血T淋巴细胞水平、血清IgA水平、辅助性T细胞相关细胞因子水平,同时比较临床疗效。结果：对照组治疗有效率（70.73%）低于研究组治疗有效率（85.37%）,具有统计学意义（P〈0.05）;与对照组比较,研究组治疗后血清Scr、BUN水平较低,Ccr水平较高,治疗后24 h UTP、β2-MG、α1-MG、尿RBC计数水平较低,治疗后外周血CD4＋T淋巴细胞、CD4＋/CD8＋及血清IgA水平较低,CD8＋T淋巴细胞水平较高,治疗后血清IL-4水平较低,IFN-γ/IL-4水平较高,具有统计学意义（P〈0.05）。结论：益气养阴通络法能够明显改善气阴两虚型IgA肾病患者肾功能,减轻血尿及蛋白尿症状,抑制自身免疫性反应,纠正Th1/Th2失衡,临床治疗效果确切。
Objective： To investigate the clinical effect of treatment of Yiqi Yangyin Tongluo method on Qi and yin deficiencytype of IgA nephropathy. M e t h o d s ： A total of 1 6 4 patients with Qi and yin deficiency type of IgA nephropathy from the Department of Nephrology in our hospital were selected and divided into 2 groups. Eighty-two cases in the control group were treated with conventional treatment of Western Medicine and the other 82 cases in the experiment group received Yiqi Yangyin Tongluo Decoction on the basis of the control group. The renal function index,urine index,peripheral blood T lymphocyte level,serum IgA level,helper T cell related cytokines levels,and the clinical effect were compared after the treatment. Results： The effective rate in the control group（ 70. 73%） was lower than the experiment group’s（85. 37%） with significant difference（ P 〈 0. 05）. Compared with the control group,serum levels of Scr and BUN were lower and Ccr level was higher in the experiment group after treatment. Levels of 24 h UTP,β2-MG,α1-MG and urine RBC count were lower after treatment. The peripheral blood levels of CD4＋ T lymphocyte,CD4＋/CD8＋ and serum level of IgA were lower. The level of CD4＋ T lymphocyte was higher after treatment. The serum level of IL-4 was lower and level of IFN-/IL-4 was higher after treatment with significant difference（ P 〈 0. 05）. Conclusion： Yiqi Yangyin Tongluo Decoction can significantly improve the renal function of patients with Qi and yin deficiency type of IgA nephropathy,reduce proteinuria and hematuria symptoms and inhibit autoimmune reaction,correct Th1/Th2 imbalance and the clinical curative effect was exactly.
