血同型半胱氨酸、颈动脉粥样硬化与原发性高血压患者24小时血压变异性的关系 被引量:16

Relationship between blood homocysteine, carotid atherosclerosis in patients with essential hypertension and 24 hours blood pressure variability

摘要 目的 探讨血同型半胱氨酸(Hcy)、颈动脉粥样硬化与原发性高血压患者24h血压变异性(BPV)(收缩压、舒张压)之间的关系.方法 选取我院原发性高血压患者120例,根据血Hcy水平的不同分为3组:A组36例(Hcy< 15.0 μmol/L),B组62例(15.0 μmol/L≤Hcy<20.0 μmol/L),C组22例(Hcy≥20.0 μmol/L).分别测定其颈动脉厚度(IMT)、24h收缩压及舒张压、脉压标准差(SD).结果 三组血压变异性随着Hcy的增高而增大,B组明显高于A组[(14.6±4.6) μmol/L比(12.6±3.4) μmol/L],C组明显高于B组、A组[(18.0±3.6)μmol/L比(14.6±4.6) μmol/L比(12.6±3.4) μmol/L].IMT厚度也随之增加,B组高于A组[(1.51 ±0.15)mm比(1.30±0.19)mm],C组高于B组、A组[(1.82±0.6)mm比(51.51 ±0.15)mm比(1.30±0.19)mm],组间比较差异有统计学意义(P<0.05).相关性分析提示,血Hcy水平与24 h BPV(收缩压、舒张压)之间存在相关性(r=0.570、0.512,P<0.01).IMT与24 h BPV呈直线相关(r=0.653、0.635,P<0.01),IMT与Hcy水平也呈直线相关(r=0.1252,P<0.01).结论 血压变异性增高可能与高Hcy及IMT相关,Hcy增高可能是造成动脉粥样硬化的重要因素. Objective To investigate the blood homocysteine (Hcy), carotid atherosclerosis in patients with essential hypertension and 24 hours blood pressure variability(BPV)(systolic, diastolic ) relationship between. Methods From 120 patients with essential hypertension, depending on the blood Hey levels, divided into three groups: A group of 36 cases (Hcy〈15.0 μmol/L), group B of 62 cases (15.0 μmol/L〈Hcy〈20.0 μmol/L), 22 cases in group C (t〉20.0 μmol/L) were measured carotid artery thickness(IMT) with 24-hour systolic and dias- tolic blood pressure, pulse pressure standard deviation (SD). Results Three groups of blood pressure variability with Hcy increased variability increases, group B was significantly higher than that in group A[ (14.6±4.6)μmol/L vs (12.6±3.4)μmol/L], group C was significantly higher than group B, and group A [(18.0±3.6)μmol/L vs (14.6±4.6)μmol/L vs (12.6±3.4)μmol/L ]. IMT thickness also increased between the two groups group B was sig- nificantly higher than that in group A[ (1.51±0.15)ram vs (1.30±0.19)mm 1, group C was significantly higher than group B,and group A[(1.82+0.6)mm vs (51.51±0.15)mm vs (1.30±0.19)mm](P〈0.05), statistically significant, correlation analysis showed that serum Hey levels with 24-hour BPV(systolic blood pressure, diastolic blood pres- sure between) correlation (r=0.570,0.512, P〈0.01). IMT with 24-hour BPV linear correlation (r=0.653,0.635, P〈0.01 ) IMT and Hcy levels also showed a linear correlation (r=0.1252, P〈0.01 ). Conclusion The increased blood pressure variability may be associated with high Hcy and 1MT-related, Hcy may cause increased an impor- tant factor for atheroselerosis.
