超声弹性成像技术在乳腺肿块检查中的价值 被引量:17

The value of ultrasound elasticity imaging in the examination of breast masses

摘要 目的探讨超声弹性成像技术对乳腺良恶性肿块的诊断价值。方法对352例乳腺肿块患者进行多种超声技术的检查,并对肿块良恶性进行判断,最后与术后病理结果进行对照,对超声检查技术进行分析。结果352例乳腺肿块患者中良性肿块289例,恶性肿块63例。彩色多普勒血流成像检查与病理诊断相同数为320例,彩色多普勒血流成像检查对乳腺肿瘤的敏感性为87.30%(55/63),特异性为91.70%(265/289),准确性为90.91%(320/352);弹性成像评分做出的良恶性诊断与病理诊断相同数为332例,其对乳腺肿瘤敏感性为93.65%(59/63),特异性为94.46%(273/289),准确性为94.32%(332/352),均高于彩色多普勒血流成像,两者相比差异有统计学意义。结论超声弹性成像技术对乳腺肿块的良恶性判定优于多普勒血流技术,可以提高对乳腺肿物诊断的敏感性、特异性及准确性。 Objective To investigate the value of ultrasound elastography in diagnosis of benign and malig-nant breast tumors. Methods 352 patients with breast masses were examined and diagnosed by a variety of ultrasound technology. Then, the results of the different ultrasonic examinations were compared with the postoperative pathological findings. Finally, the efficiency of different ultrasound technologies was ana- lyzed. Results In 352 cases of breast tumor patients, 289 cases were benign tumors and 63 cases were ma- lignant tumors in. The results of 320 cases from color Doppler flow imaging agreed well with the results of pathological diagnosis, so the sensitivity of color Doppler flow imaging in the diagnosis of breast tumor was 87.30% (55/63) and the specificity was 91.70%(265/289) and the accuracy was 90.91% (320/352); The results of 332 cases from elastography diagnosis supported the results of pathological diagnosis, so the sensitivity of elastography diagnosis was 93.65% (59/63) and the specificity was 94.46% (273/289) and the accuracy was 94.32% (332/352), which were higher than those of the color Doppler flow imaging and the differences were significant. Conclusion Ultrasound elastography in the identification on benign and malignant tumors of breast is better than Doppler flow technology, which can improve the sensitivity, the specificity and the accuracy in diagnosis of breast tumor.
