摘要
目的:探讨FOCUS超声刀在脾虚气陷型中重度贫血痔手术中的应用价值。方法:将145例脾虚气陷型混合痔出血继发中重度贫血的患者采用随机数字表的方法分为超声刀组72例及传统电刀组73例,从手术时间、术中出血量、术后VAS疼痛评分、术口水肿评分、术后住院时长及并发症(术后继发性大出血及尿潴留)发生情况进行观察分析。结果:在手术时间和术中出血量方面,超声刀组具有优势,组间比较,差异有统计学意义(P<0.05)。超声刀组的术后住院天数也整体小于传统电刀组。在术后疼痛、水肿这两个与患者自我感受最密切相关的因素方面,超声刀组患者术后早期的疼痛评分要低于传统电刀组,差异有统计学意义(P<0.05);术后第1天,2组患者的水肿情况基本相同(P>0.05),而第2、3天传统电刀组患者的术口水肿程度要甚于超声刀组(P<0.05);2组的术后并发症情况方面比较,差异无统计学意义(P>0.05)。结论:FOCUS超声刀凭借其优越的止血特点,可以用于脾虚气陷型混合痔出血导致的中重度贫血患者的手术治疗,具有快速、有效、安全的特点。
Objective：To observe the application value of FOCUS ultrasonic scalpel in treating patients with spleen qi stagnation type of moderate and severe anemia hemorrhoids.Methods：Divided 145 cases of patients with spleen qi stagnation type of mixed hemorrhoids bleeding secondary moderate to severe anemia into the ultrasonic scalpel group with 72 cases and the conventional electrotome group with 73 cases randomly.Observed and analyzed operation time,intraoperative blood loss,postoperative pain scores of visual analog scale（VAS）,postoperative edema scores,postoperative hospitalization time and complications（secondary hemorrhage after operation and urinary retention）.Results：In terms of operation time and intraoperative blood loss,ultrasonic scalpel group had advantages,differences being significant between the two groups（P〈0.05）.Hospitalization time in ultrasonic scalpel group was generally shorter than that in conventional electrotome group.In terms of postoperative VAS pain scores and postoperative edema scores（two factors that were most closely related to self perception）,postoperative pain scores at early stage in ultrasonic scalpel group were lower than those in conventional electrotome group,differences being significant（P〈0.05）.On the first day after operation,edema in both groups was nearly the same（P〉0.05）.On the second and third day,edema after operation in conventional electrotome group was more serious than that in ultrasonic scalpel group（P〈0.05）.In terms of complications in both groups,no significant difference was observed（P〉0.05）.Conclusion：FOCUS ultrasonic scalpel can be applied for the operative treatment for patients with spleen qi stagnation type of mixed hemorrhoids bleeding secondary moderate to severe anemia by virtue of its excellent characteristic of hemostasis.It is quick,effective and safe.
