摘要
本文提出的肝纤维化/肝硬化的"正虚血瘀"中医病机已被学术界接受,病机中"血瘀"的阐述比较明确,但是有关"正虚"的阐述则比较笼统,未能阐明正虚的具体表现,因此较难指导临床以进一步提高疗效。本文综述了课题组近年来为解答这一问题开展的临床观察和研究的结果。明确指出肝纤维化/肝硬化患者"正虚血瘀"病机中,"正虚"的具体表现是脾气虚弱和肝肾阴虚,因阴精虚损使疾病进展,致"虚损生积"导致肝硬化;进而提出在临床应用活血化瘀药的同时,需根据证候表现采用健脾益气或补益肝肾之法,尤其要重视及早养阴柔肝以延缓或逆转疾病的进展。
＂Zhengxu Xueyu＂, a TCM pathogenesis of hepatic fibrosis / cirrhosis referred in this study, has been hitherto accepted in the domestic academic community. It was clearly that ＂Xueyu＂ signified the obstruction of blood in the liver, while the concept of ＂Zhengxu＂ remained ambiguity. It was indicated that curative effect would not be improved if the unclear theory of TCM pathogenesis had been used to guide the clinic practice. In this review, we had carried out several clinical observations and researches for perfecting the TCM pathogenesis recent years and aimed to deal with the results. It was specified that ＂Zhengxu＂ referred to the insufficiency of the spleen and deficiency of the liver-yin and kidney-yin in the patients suffered from hepatic fibrosis / cirrhosis. The asthenic or damaged yin fluid in the liver and kidney would lead to cirrhosis in fibrotic patients. So it was suggested as Chinese herb medicine was adopted in the studies of disperse blood stasis syndrome in clinic, that the herbs for invigorating spleen and supplementing qi and for tonifying the liver-yin and kidney-yin need selecting according to syndrome manifestation. It is extremely significant to nourish yin and liver for the treatment of hepatic fibrosis / cirrhosis to defer its progress or reverse the disease at its early stage.
