摘要
复法制方是中医辨治急难病证的特色制方思路,在梳理古今医家对复法制方经验源流的基础上,文章介绍了国医大师周仲瑛基于复合病机理论临床常用辨治疑难杂病的学术经验。以肝炎后肝硬化为例,结合“湿热瘀毒郁结,肝脾肾亏虚”的基本病机及在病情进展中出现本虚标实,虚实兼夹转化的病机特点,临床采用复合立法,通过对升降结合、寒热并用、表里相合、敛散相伍、多脏同治复法等思路的具体运用,总结了名老中医复法治疗肝硬化的辨证思路及治法方略。展现出复法制方具有邪正兼顾、气血并调、阴阳并治等“杂合以治,各得其所宜”的疗效优势,善用复方大法体现了名医“机圆法活,知常达变”的辨治思路及对中药性味熟稔于心,丰富缜密的用药经验。
The multiple method for making prescriptions is the characteristic thought of differentiating and treating critical patients in traditional Chinese medicine,this paper summarized the legal origin clinic experience of multiple method for making prescriptions and discussed the academic thought of compound pathogenesis commonly-used for treating critical patients by professor ZHOU Zhongying.Take hepatocirrhosis for example,combined with the basic pathogenesis of“dampness,heat,blood stasis and toxin stagnation,deficiency of liver,spleen and kidney”and the pathogenesis characteristics of deficiency and excess,transformation of deficiency and excess in the progress of the disease,the multiple method for making prescriptions is adopted in clinic.Through the specific application of the ideas of combination of descending and ascending,combination of cold and heat,combination of the exterior and the interior,combination of convergence and dispersion,simultaneous treatment and recovery of multiple Zang-Fu organs,this paper summarized the syndrome differentiation ideas and treatment strategies of famous and old traditional Chinese medicine physicians in the treatment of hepatocirrhosis.It shows that the multiple method for making prescriptions has the curative effect advantages of“heterozygous treatment,appropriate for each”such as giving consideration to both evil and vital Qi,regulating Qi and blood,and treating Yin and Yang together.The good use of multiple method for making prescriptions reflects the famous doctor’s syndrome differentiation and treatment idea of“flexible treatment,gasping the changes of the disease”,as well as being familiar with the nature and tastes of traditional Chinese medicine and rich and meticulous medication experience.
