摘要
目的:了解武汉地区血管紧张素转换酶抑制剂(angiotensin converting enzyme inhibitor,ACEI)的使用情况,探讨ACEI的临床应用现状和趋势,为临床合理用药提供参考。方法:采用金额排序法和用药频度(defined daily dose system,DDDs)排序法对武汉市32家医院2012—2014年ACEI的使用数据进行统计分析。结果:ACEI的总销售金额呈逐年升高趋势,培哚普利销售金额增幅较大,而依那普利、喹那普利、雷米普利及赖诺普利的销售金额则有不同程度的降低。贝那普利、培哚普利和福辛普利使用率最高,其DDDs排序与其销售金额排序保持同步,居前3位;其中,培哚普利的DDDs增速最快,2014年其DDDs排序已居第1位,而依那普利作为最经济的ACEI仍然受到医师和患者的青睐。结论:短效ACEI的市场逐渐萎缩,但因其独特的肾功能保护作用,以培哚普利为代表的长效ACEI的使用不断增长,这为更多新型长效ACEI的研发提供了方向。
OBJECTIVE： To investigate the application of angiotensin converting enzyme inhibitor（ ACEI） in Wuhan,and to probe into the clinical application status and trend of ACEI,so as to provide reference for the rational drug use in clinic. METHODS： Statistical analysis was conducted on the application data of ACEI in 32 Hospitals in Wuhan during 2012-2014 according to the consumption sum and defined daily dose system（ DDDs） ranking methods.RESULTS： The total consumption sum of ACEI were in a increasing trend year by year,and the ascent range of perindopril was significant,yet the consumption sum of enalapril,quinapril,ramipril and lisinopril were decreased to different degree. The application rate of benazepril,perindopril and fosinopril was the highest,and the DDDs ranking kept pace with the consumption sum,which dominated the top 3 places. Meanwhile,the speed-rasing of perindopril was the fastest,which took the lead of DDDs ranking in 2014; and as the most economical ACEI,enalapril still preferred by physicians and patients. CONCLUSIONS： The market of short-acting ACEI has shrunk gradually; but for its unique protection for renal function,the long-acting ACEI like perindopril shows an increasing application,which provide reference for the development of new long-acting ACEI.
