摘要
〔目的〕探讨类风湿关节炎患者血清中RF和TNF-α在诊断及临床疗效评价中的意义。〔方法〕选取2013年1月至2015年1月开封市第二人民医院收治的RA患者128例,根据DAS28评分及临床症状分为RA活动期患者(66例)、RA缓解期患者(62例)作为试验组;随机选取门诊健康就诊者60例作为对照组。检测RA活动组、缓解组、对照组患者的RF抗体和TNF-α等各项指标,进行统计学分析。〔结果〕RA活动组和缓解组患者血清的RF和TNF-α水平均高于健康对照组。RA活动组血清TNF-α水平明显高于缓解组,差异有统计学意义(P<0.05)。128例RA组患者中RF和TNF-α的灵敏度分别为80.5%、72.2%,特异性分别为71.2%,90.5%,TNF-α和RF联合检测的灵敏度和特异性分别为71.2%、96.7%,特异性较两项单独检测时明显提高。TNF-α与RA的DAS28评分成正相关,差异有统计学意义(P<0.01)。〔结论〕联合检测RA患者血清中的RF和TNF-α水平对患者的治疗效果和病情监测有重要临床价值。
〔Objective〕To investigate the application of combined detection of two serum markers, RF and TNF-α for the diagnosis and therapeutic evaluation of RA.〔Methods〕Serum samples were randomly obtained from 128 RA patients and 60 normal controls in Kaifeng Second People＇s Hospital from January 2013 to January 2015. The patients were divided into two groups including 66 active RA patients and 62 inactive RA patients. Levels of RF、TNF-α were measured. The values of 2 serum biomarkers and their varied combinations for RA diagnosis were systemically assessed. 〔Results〕The RF and TNF-α level of RA patients were higher than healthy controls. The TNF-α level of RA activity group was significantly higher than the level of inactive group. The specificity of combined detection of RF and TNF-α was 96.7% which were higher than single detection. RA patients with serum TNF-α levels and DAS28 disease activity score was positively correlated（ r = 0.601, respectively P 0.01）.〔Conclusion〕Combined detection of RF and TNF-α had high diagnostic value and contributed to estimate the clinical observation.
