摘要
目的探讨机械通气对治疗急性肺损伤动物模型中TNF-α表达的影响。方法选用40只SD大鼠,制作ALI模型,随机分为LV、HV、LV2P、LV5P、LV8P 5个组;通气2 h后处死动物,采集血及支气管肺泡灌洗液(BALF),用放射免疫法测定其中TNF-α浓度。结果血清TNF-α浓度变化:HV组明显高于LV组(P<0.01),小潮气量组加用PEEP各组中,LV2P组明显高于LV5P和LV8P组(P<0.01);支气管肺泡灌洗液中TNF-α浓度变化:HV组明显高于LV组(P<0.01),小潮气量组加用PEEP各组中,LV8P组明显高于LV2P组和LV5P组(P<0.01)。结论小潮气量及采用PEEP机械通气对肺组织有保护性作用。
OBJECTIVE To investigate the effect of ventilation on the changes in TNF-α concentration of BALF on acute lung injury model. METHODS Forty healthy male Sprague-Dawley rats were randomly divided into five groups LV, HV, LV2P, LV5P, LV8P and the rat model of acute lung injury was built. Mechanical ventilation was employed in the test groups. Rats were sacrificed after 2 hours ventilation when the serum acquired. The supernatant of BALF was detected for TNF-α concentration. RESULTS The changes in TNF-α content of serum: the TNF-α contentration of serum was higher in HV group than that in LV group(P〈0.01 ). It was higher in LVZP group than that in LV5P and LV8P groups (P〈0.01). The changes in TNF-α content of BALF: The TNF-α content of BALF was higher in HV group than that in LV group (P〈0.01). It was higher in LV8P group than that in LV2P and LV5P groups (P〈0.01). CONCLUSIONS The low tidal volume and with PEEP ventilation have protective effects.
