乳腺超声与钼靶摄影在乳腺疾病诊断中的临床应用及危险因素分析 被引量:7

摘要 目的:探讨超声与钼靶摄影或两者联合在乳腺疾病诊断中的临床应用及危险因素。方法:选取乳腺癌患者75例,采用超声和钼靶摄影或两者联合对乳腺进行检查,比较三种方法的敏感性、特异性及准确率,以受试者工作特征曲线下面积(AUC)表示不同方法对乳腺癌的诊断效果,采用Logistic回归分析筛选乳腺癌的危险因素。结果:超声联合钼靶摄影对乳腺癌的诊断敏感性、特异性及准确率分别为92.31%、91.94%和92.00%,高于单一超声或钼靶摄影检查,差异有统计学意义(P<0.05);虽然超声检查的敏感性、特异性及准确率比钼靶摄影高,但差异无统计学意义(P>0.05)。超声联合钼靶X线摄影的检测准确性最高,AUC最大值分别为0.75、0.77、0.79。年龄、家族史、体重指数(BMI)、未生育、未哺乳、高脂饮食是乳腺癌发生的危险因素。结论:乳腺超声与钼靶摄影均能较好的诊断乳腺癌,两者结合检出率更高。乳腺癌的发生与年龄、肿瘤家族史、BMI、未生育、未哺乳、高脂饮食有关。 Objective: To explore the clinical application and risk factors of breast ultrasounography and motarget mammography in the diagnosis of breast disease.Method: 75 cases with breast cancer were underwent breast ultrasounography and mo-target mammography detection. The sensitivity, specificity and accuracy were compared between three methods. The diagnose effect of three methods were tested by area under the curve(AUC), and the logistic regression analysis was adopted for study.Result:The sensitivity, specificity and accuracy were respectively 92.31%,91.94% and 92.00% by breast ultrasounography and mo-target mammography,they were higher than only Breast ultrasounography or mo-target mammography,the differences were statistically significant(P〈0.05); there was no significant difference between breast ultrasounography and mo-target mammography(P〉0.05). The maximum AUC of three methods were respectively 0.75,0.77,0.79. Ultrasounography joint mo-target mammography had the highest detection accuracy. Logistic regression analysis showed that age, family history of cancer, BMI, nulliparous, not lactation,and high-fat diet were the risk factors of breast cancer.Conclusion:Both ultrasound and Mo-target mammography have good effect in the diagnosis of breast disease, but combined ultrasound's inspection result with mo-target mammography's result can improve diagnosis accurate rate significantly. The breast cancer is related to age, family history of cancer, BMI, nulliparous, not lactation and high-fat diet.
