摘要
目的观察合并月经失调的HIV/AIDS患者的证候分布,探讨中医证候特点。方法选取月经失调的HIV/AIDS患者141例,基于因子分析的方法提取证候要素,归纳可能提示的证候。结果症状体征分布特点:主要症状以乏力为主,次要症状以自汗、盗汗、恶心、气短为主,体征以皮疹为主。因子分析结果得出8个公因子,症状组合提示的证候要素有12个,其中病位类要素有脾、肾、肝、肺,病性类要素有气虚、阴虚、阳虚、气滞、血瘀、痰、湿、热。提示的常见证候有9个,为脾气虚弱、气虚血瘀、脾肾阳虚、痰瘀互结、肝经郁热、气滞痰阻、湿热蕴结、气阴两虚、肝火犯肺。结论合并月经失调的HIV/AIDS患者有自己的证候特点,在临床上应予以针对性的辨证治疗。
Objective: To observe the distribution of syndromes in HIV/AIDS patients with complicated menstrual disorders and to explore the characteristics of TCM syndromes. Methods: A total of 141 HIV/AIDS patients with menstrual disorders were selected. Factor analysis was used to extract syndrome elements and to summarize the syndromes that may be suggested. Results: The distribution characteristics of symptoms and signs were that the main symptoms were mainly fatigue and the secondary symptoms were mainly spontaneous sweating,night sweats,nausea and shortness of breath. The signs were mainly rash. The results of factor analysis showed 8 common factors,and the symptom combination showed 12 syndrome elements. Among them,the disease-like elements included spleen,kidney,liver and lung and the pathological elements had qi deficiency,yin deficiency,yang deficiency,qi stagnation,blood stasis,sputum,wetness and heat. The were 9 suggested common syndromes,namely,spleen weakness,qi deficiency and blood stasis,spleen and kidney yang deficiency,phlegm and stasis combination,stagnated heat of liver channel,stagnation of qi causing phlegm retention,accumulation of damp heat,qi and yin deficiency and lung attacked by liver fire.Conclusion: HIV/AIDS patients with complicated menstrual disorders have their own syndrome characteristics,and should be targeted for clinical treatment.
