乳胶增强免疫比浊法测定血清肌钙蛋白I的实验探讨 被引量:7

摘要 目的探讨胶乳增强免疫比浊法定量测定心肌肌钙蛋白Ⅰ(cTnI)的可靠性。方法应用胶乳增强免疫比浊法检测cTnI。通过对测定过程的吸光度观察分析,探讨标本与试剂比例,制作校准曲线,进行线性试验、精密度试验、回收试验:同时随机选取健康体检者47例与心肌疾病患者41例测定cTnI及心肌酶谱进行临床应用评价。结果用胶乳增强免疫比浊法测定cTnI标本与试剂最适比例为1:10.5;cTnI在0.3~25.0ng/ml测定范围内具有良好线性;批内精密度为1.5%-4.8%;批间精密度为2.8%~6.5%;回收率为94.3%-105.7%;cTnI及心肌酶谱在心肌疾病组与健康对照组差异均有统计学意义(P〈0.01),敏感性cTnI为90.3%,心肌酶谱中的其他几项指标仅为41.9%~71.0%。结论通过本次实验探讨,确定胶乳增强免疫比浊法测定cTnI的试验参数及性能评价指标,说明本法线性范围较宽,精密度及准确性良好,回收率符合要求,适合实验室开展。 Objective To explore the reliability of latex-enhanced immunoturbidimetry in the detection of serum cardiac tro-ponin Ⅰ (eTnI). Methods The serum eTnI was tested by latex-enhanced immunoturbidimetry. The proportion of specimen and re-agent, making calibration curve,linear test, precision test and recovery test were assessed by absorbance measurement process. The levels of cTnI and myocardial enzymes of 47 healthy individuals and 41 patients with cardiac disease randomly were detected and analyzed by for clinical evaluation. Results The optimal specimen and reagent proportion of latex-enhanced immunoturbidimetry for cTnI test was 1 : 10.5 ; cTnI had good linear measurement range within 0.3 - 25.0 ng/ml ; within-run precision was 1.5% - 4. 8% , inter-assay precision was 2.8% -6.5% ;recovery rate was 94.3% - 105.7% ;the difference in the levels of eTnI and myo-cardial enzymes between the myocardial disease group and the healthy control group was statistically significant( P 〈0.01 ). The sensitivity of eTnl was 90.3% , several other indicators of myocardial enzymes was only 41.9% - 71.0%. Conclusion The pa-rameter and performance evaluation index of cTnI assay by latex-enhanced immunoturbidimetry were confirmed. This method has wider linear range, higher precision, better accuracy, recovery fill requirement and suit for laboratory.
