摘要
目的探讨2型糖尿病合并缺血性脑卒中患者血清核因子(NF-κB)、白细胞介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)的变化以及临床意义。方法 24例2型糖尿病伴急性缺血性脑卒中患者为糖尿病+脑缺血组,22例非2型糖尿病但伴急性缺血性脑卒中患者为脑缺血组,另有26名健康体检者为对照组,检测前2组脑卒中发生后不同病变时期(0、1、2、3 d及1、4、26周)外周血NF-κB、IL-6、TNF-α水平,并与对照组比较,观察前2组患者上述细胞因子与缺血性脑卒中、糖尿病病情及预后的关系。结果糖尿病+脑缺血组血清NF-κB、IL-6、TNF-α水平与病情呈正相关(r=0.52、0.61、0.55),从发病当天到1周时,各因子水平较脑缺血组和对照组均显著升高(P<0.05);随着病情缓解,到发病26周时,上述因子水平逐渐下降至接近正常水平。结论 2型糖尿病合并急性缺血性脑卒中患者血清NF-κB、IL-6、TNF-α表达水平与疾病发生、发展和转归密切相关;血管损伤、炎症因子释放可能在患者的发病过程中具有重要作用。
Objective To investigate the changes of nuclear factor-kappa B (NF-κB), interleukin-6 (IL-6), and tumor necrosis factor-α (TNF-α) in pathogenesis of type 2 diabetes mellitus with ischemic stroke and its clinical significance. Methods Twen- ty-four cases of type 2 diabetes mellitus coupled with acute ischemic stroke were assigned to the diabetes mellitus + cerebral ischemia group, 22 cases of non-type 2 diabetes mellitus coupled with acute ischemic stroke were assigned to the cerebral ischemia group, and another 26 healthy subjects to the control group. Changes in the serum levels of NF-κB, IL-6, and TNF-α were detected in the first 2 groups of patients with ischemic stroke at different time points ( d 0, d 1, d 2, d 3 and w 1, w 4, w 26) , and then were compared with those of the control group. The relationship between the above cytokines and ischemic stroke was determined. Finally, observations were made on the relationship between the above cytokines and ischemic stroke, the state of diabetes mellitus and prognosis. Results Serum levels of NF-κB, IL-6, and TNF-ct in the patients of the diabetes mellitus + cerebral ischemia group were positively correlated to the state of illness. Levels of various cytokines from the onset of the day to week 1 elevated significantly, when compared with those of the cerebral ischemia group and the control group (P 〈 0.05 ). Following relief of the illness, levels of the above cytokines gradually decreased half a year later following the onset of the illness and finally came near the normal levels. Conclusion The expression levels of NF-κB, IL-6 and TNF-α in the patients with type 2 diabetes mellitus coupled with acute ischemic stroke were closely correlated with the occurrence, development and prognosis of the disease. Vascular lesion and release of inflammatory factors might play an important role in the course of the disease.
