摘要
目的探讨针灸联合低频重复经颅磁刺激(rTMS)对老年脑梗死患者肢体功能、神经功能及炎性因子的影响.方法选取2020年9月—2021年9月我院收治的老年脑梗死患者109例,随机分为观察组(n=55)和对照组(n=54),对照组采用低频rTMS治疗,观察组采用针灸联合低频rTMS治疗,均持续治疗8周.治疗前后,分别采用Fugl-Meyer运动量表(FMA)和美国国立卫生研究院卒中量表(NIHSS)评估两组患者的肢体功能、神经功能,并采用Barthel指数(BI)评估患者的日常生活活动能力;同时比较两组患者的血清C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)等炎性因子水平和神经元特异性烯醇化酶(NSE)、髓鞘碱性蛋白(MBP)水平.结果治疗后,观察组的FMA上肢、下肢评分及BI评分均高于对照组,NIHSS评分低于对照组,组间差异有统计学意义(P<0.05).治疗后,观察组的血清CRP、TNF-α、IL-6水平和NSE、MBP水平均明显低于对照组,组间差异有统计学意义(P<0.05).结论针灸联合低频rTMS能够明显改善老年脑梗死患者的肢体功能和神经功能,抑制炎性反应,且能有效降低患者血清NSE、MBP水平,值得临床推广使用.
Objective To investigate the effects of acupuncture combined with low frequency repeated transcranial magnetic stimulation(rTMS)on limb function,nerve function and inflammatory factors in elderly patients with cerebral infarction.Methods A total of 109 elderly patients with cerebral infarction admitted to our hospital from September 2020 to September 2021 were randomly divided into an observation group(n=55)and a control group(n=54).The control group was treated with low frequency rTMS,and the observation group was treated with acupuncture combined with low frequency rTMS.The treatment lasted for 8 weeks in both groups.Before and after treatment,the Fugl-Meyer assessment(FMA)and National Institutes of Health Stroke Scale(NIHSS)were used to evaluate the limb function and neurological function of the two groups,and the Barthel index(BI)was used to evaluate the ability of daily living activities of the two groups.Meanwhile,the serum levels of c-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and neuron specific enolase(NSE)and myelin basic protein(MBP)in the two groups were compared.Results After treatment,the FMA scores of upper limbs,lower limbs and BI scores of the observation group were higher than those of the control group,while NIHSS score was lower than that of the control group,the differences between the groups were statistically significant(P<0.05).After treatment,the levels of CRP,TNF-α,IL-6,as well as the levels of NSE and MBP in the observation group were significantly lower than those in the control group,the differences between the groups were statistically significant(P<0.05).Conclusion Acupuncture combined with low frequency rTMS can significantly improve the limb function and nerve function of elderly patients with cerebral infarction,inhibit inflammatory response,and effectively reduce the levels of serum NSE and MBP,which is worthy of clinical application.
