慢性阻塞性肺疾病急性加重期患者血清中IL-17、MMP-9水平的变化及意义 被引量:12

The changes and significance of IL-17 and MMP-9 in serum in patients with chronic obstructive pulmonary disease during acute exacerbation

摘要 目的通过检测慢性阻塞性肺疾病(COPD)急性加重期患者治疗前后血清中白细胞介素-17(IL-17)、基质金属蛋白酶-9(MMP-9)水平,对IL-17、MMP-9在COPD中的变化及其与肺功能的相关性进行研究。方法选择COPD急性加重期患者45例(COPD组),并按肺功能分级分为Ⅰ级(轻度组)、Ⅱ级(中度组)、Ⅲ级(重度组)三组,每组各15例,及健康自愿者20例(对照组),采用双抗夹心ELISA法,对各组COPD患者入院时及治疗10~15d病情缓解后的血清IL-17、MMP-9含量进行测定,并同步行肺功能及相关检查,将各组所测结果与对照组进行对比分析。结果 (1)COPD各组治疗前后血清IL-17、MMP-9含量均高于对照组,且治疗后IL-17、MMP-9水平较治疗前下降(P<0.05);COPD各组治疗前后IL-17、MMP-9水平随分级增加而增高,其中中度组、重度组治疗前后IL-17水平与轻度组之间差异有统计学意义(P<0.05),轻度组治疗前后MMP-9水平与重度组之间差异有统计学意义(P<0.05)。(2)COPD组治疗前后FEV1%Pred、FEV1/FVC%、MMEF%Pred、V50%Pred、V25%Pred明显低于对照组,经治疗后COPD组各项肺功能指标均升高(P<0.05)。(3)COPD患者治疗前血清IL-17与血清MMP-9水平呈正相关;COPD急性加重期患者治疗前血清IL-17与肺功能指标FEV1%Pred、FEV1/FVC%、MMEF%Pred呈负相关,血清MMP-9与FEV1%Pred、FEV1/FVC%、MMEF%Pred、V50%Pred、V25%Pred均呈负相关。结论 COPD组患者在病情急性加重期和治疗缓解后血清IL-17、MMP-9水平均升高,并与疾病严重程度及气流受限程度有关,提示IL-17、MMP-9在COPD气道炎症及重塑中可能具有重要作用。 Objective To study the relationship of lung function and the changes of interleukin (IL)-17, matrix metalloproteinase (MMP)-9 in serum of patients with chronic obstructive pulmonary disease (COPD) during acute exacerbation before and after treatment by detected the value of serum. Methods 45 inpatients with acute exacerbation of COPD (COPD group) and 20 healthy volunteers (control group) were included in this study. COPD patients were divided into groups according to lung function which were 15 grade I (mild group), 15 grade II (moderate group), and 15 grade III (svere group). The serum samples were collected from COPD patients before and after 10-15 days therapy. The concentrations of IL-17, MMP-9 in serum were measured by enzyme-linked immunosorbent assay (ELISA). Meanwhile the lung function and related inspection items were also measured. The results of patients were compared with those of healthy volunteers. ResultsThe serum concentrations of IL-17, MMP-9 in COPD various groups before and after treatment were significantly higher than those in control group, and after treatment, the levels of IL-17 , MMP-9 were decreased compared with before (P〈0.05). The serum levels of IL-17 , MMP-9 in COPD various groups before and after treatment had a raised tendency with the increase of grade, the differences of IL-17 between moderate group and mild group/severe group and mild group were obvious (P〈0.05), the differences of MMP- 9 between severe group and mild group were obvious (P〈0. 05). The levels of FEV1 %Pred, FEV1/FVC%, MMEF%Pred, V50 %Pred, and V25 %Pred in COPD group before and after treatment were obviously lower than those in control group (P〈0.05), after treatment, the levels of FEV1 %Pred, FEV1/FVC%, MMEF%Pred, V50 % Pred, and V25% Pred were all improved compared with before (P(0.05). There were a positive correlation between the concentrations of IL-17 and the concentrations of MMP-9 in patients with COPD during acute exacerbation. The levels of IL-17 in patients with COPD during acute exacerbation negatively correlated with those of FEV1% Pred, FEV1/FVC%, MMEFMPred. Th, e levels of MMP-9 negatively correlated with those of FEV1% Pred, FEVI/ FVC%, MMEF%Pred, V50 %Pred, V25 %Pred. Conclusion The levels of IL-17, MMP-9 in patients with COPD both during acute exacerbation and after treatment were significantly increased, which were associated with the severity of disease and airflow limitation. IL-17, MMP-9 may play a important role in the airway inflammation and remodeling of COPD.
