摘要
目的观察补虚平喘膏穴位敷贴对Ⅲ级和Ⅳ级慢性阻塞性肺疾病稳定期患者细胞免疫和BODE评分指数的影响。方法门诊确诊Ⅲ级和Ⅳ级COPD稳定期患者共61例,分为2组,分别给予吸入舒利迭或舒利迭联合补虚平喘膏穴位敷贴,治疗12周,观察治疗前后CD+3、CD+4、CD+8、CD+4/CD+8、BODE指数评分的变化。结果治疗后联合组CD+3、CD+4、CD+4/CD+8升高,CD+8降低,舒利迭组CD+4/CD+8升高,CD+8降低,联合组较舒利迭组CD+4/CD+8升高更显著。2组BODE指数评分与治疗前比较均有好转(P<0.05),联合组与舒利迭组比较改善更显著。治疗1 a内联合组患者感冒及急性发作的次数较舒利迭组少(P<0.05)。结论舒利迭联合补虚平喘膏穴位敷贴是Ⅲ级和Ⅳ级COPD稳定期患者有效的治疗方案,能改善细胞免疫,减少急性发作次数,减缓肺功能下降速度,更好缓解临床症状。
Objective To observe the effect of point application with Buxu Pingchuan plaster on cellular immunity and BODE score index in patients with Ⅲ and IV grade stable COPD.Methods 61 outpatients were selected according to the diagnosis criteria of Ⅲ and IV grade stable COPD and assigned to two groups.Seretide was administered to all patients,and Buxu Pingchuan plaster was given to patients in the treatment group additionally.The treatment course was 12 weeks,and the numbers of CD ＋3,CD ＋4,CD ＋8,the ratio of CD＋4 and CD＋8,and BODE score index before and after the treatment were compared.Results The number of CD ＋8 was decreased,and the ratio of CD ＋4 and CD ＋8 was increased after treatment in both of two groups,and the numbers of CD ＋3 and CD ＋4 were increased in the treatment group additionally.The increasing amplitude of the ratio of CD ＋4 and CD ＋8 in the treatment group was higher than that in the control group.BODE score index was increased significantly after treatment in both of two groups（P0.05）,and the increas-ing amplitude of BODE score index in the treatment group was higher than that of control group.The times of frigorism and acute episode in the treatment group were obviously less than that of control group in one year after receiving the treatment（P0.05）.Conclusion Seretide combined with Buxu Pingchuan plaster is an effective method for the treatment of Ⅲ and IV grade stable COPD.The method could improve cellular immu-nity,reduce the times of acute episode,step down the decrease of lung function and relieve the symptoms markedly.
