摘要
目的:评价运用中西医综合疗法治疗老年骨质疏松性胸腰椎压缩性骨折的临床疗效。方法:将300例老年骨质疏松性胸腰椎压缩性骨折患者随机分为2组,每组150例。治疗组采用过伸复位法、腰部垫枕、腰背肌功能锻炼、骨折三期辨证用药、局部外敷中药、理疗及骨质疏松病因治疗等中西医综合疗法治疗;对照组采用卧床休息及对症处理、腰背肌功能锻炼。比较两组VAS评分、椎体压缩率、复位率及总体疗效。结果:治疗后1周、1个月、3个月及末次随访时VAS评分、椎体压缩率、复位率及总体疗效比较,治疗组均优于对照组,差异有统计学意义(P<0.05)。结论:过伸复位、腰部垫枕、腰背肌功能锻炼、骨折三期辨证用药、局部外敷中药、理疗及骨质疏松病因治疗等中西医综合疗法是非手术治疗老年骨质疏松性胸腰椎压缩性骨折的有效方法。
Objective：To evaluate the clinical curative effect of integration of traditional Chinese medicine and western medicine（TCM WM therapy）on the treatment of aged osteoporotic thoracolumbar compression fracture（AOTCF）.Methods：300 AOTCF patients were randomly divided into 2 groups,150 cases in each group.The patients in treatment group were administrated with hyperextension restoration,bolstering waist with pillow,functional exercise of low back muscle,medication according to differentiation of symptoms and signs during three periods after fracture,local externally application of TCM,physical therapy,osteoporosis etiology treatment and the other TCM WM therapys,while the patients in control group were administrated with bed rest,symptomatic treatment and functional exercise of low back muscle.VAS scores,vertebral compression ratio,reposition ratio and the total curative effect were compared between the 2 groups.Results：One week,one month,3 months after the treatment and the last follow-up,the VAS scores,vertebral compression ratio,reposition ratio and the total curative effect of treatment group were all better than those of control group,there was statistical difference between the 2 groups（P0.05）.Conclusion：TCM WM therapy,including hyperextension restoration,bolstering waist with pillow,functional exercise on low back muscle,medication according to differentiation of symptoms and signs during three periods after fracture,local externally application of TCM,physical therapy,osteoporosis etiology treatment and so on,performs well on the treatment of aged osteoporotic thoracolumbar compression fracture.
