摘要
目的:探讨老年骨质疏松椎体压缩性骨折(OVCF)患者采用手术联合中医补肾壮骨汤治疗的临床效果。方法:选取我院拟接受椎体成形术治疗的老年OVCF患者97例作为研究对象,采用随机数字表法分为中医组48例和常规组49例,两组患者均采用椎体成形术治疗,术后给予基础的药物治疗,中医组患者同时服用补肾壮骨汤;观察两组患者围手术期指标、手术前后的Cobb角、椎体前缘高度丢失率、Oswestry功能指数(ODI)、血清25羟维生素D(25-OH-D)、N端骨钙素(N-MID)、β胶原降解产物(β-CTX)、椎体骨密度及股骨颈骨密度的差异。结果:中医组和常规组的手术时间、骨水泥用量、手术切口长度、术后下地时间比较,差异无统计学意义(P>0.05);术前、术后3d、术后6个月,中医组和常规组的Cobb角、椎体前缘高度丢失率组间比较,差异无统计学意义(P>0.05);两组患者的Cobb角、椎体前缘高度丢失率在术后3d、术后6个月均较术前降低(P<0.05);术前、术后3d、术后6个月,中医组和常规组的ODI值组间比较,差异无统计学意义(P>0.05);两组患者的ODI值在术后3d、术后6个月均较术前降低(P<0.05);治疗前,中医组和常规组的25-OH-D、N-MID、β-CTX测定值组间比较,差异无统计学意义(P>0.05);治疗后,中医组的N-MID、β-CTX测定值均低于常规组(P<0.05);治疗前,中医组和常规组的椎体骨密度、股骨颈骨密度测定值比较,差异无统计学意义(P>0.05);治疗后,中医组的椎体骨密度、股骨颈骨密度测定值均高于常规组(P<0.05)。结论:老年骨质疏松OVCF患者采用手术联合中医补肾壮骨汤治疗有利于改善骨代谢水平,缓解骨质疏松的病情。
Objective:To explore the clinical effect of elder osteoporotic vertebral compression fractures(OVCF)patients treated with surgery combined with TCM Bushen Zhuanggu Decoction.Methods:Ninety-seven elderly patients with OVCF who were treated with vertebroplasty in our hospital were selected as the research objects.They were divided into 48 cases in a traditional Chinese medicine group and 49 cases in a conventional group by the random number table.The two groups of patients were treated with vertebroplasty.Basic medical treatment was given.Patients in the traditional Chinese medicine group took Bushen Zhuanggu Decoction at the same time.The perioperative indicators,the Cobb angle before and after the operation,the loss rate of the anterior vertebral height,the Oswestry functional index(ODI),and the serum 25 hydroxy vitamin were observed.Serum 25 hydroxyvitamin D(25-OH-D),N-terminal osteocalcin(N-MID),β-collagen degradation products(β-CTX),vertebral bone density and femoral neck bone density differences.Results:There was no statistically significant difference between the operation time,bone cement dosage,surgical incision length,and postoperative time of setting the ground between the traditional Chinese medicine group and the conventional group(P>0.05).Before operation,3 days after operation,and 6 months after operation,the traditional Chinese medicine group was no significant difference in the loss rate of Cobb angle and anterior edge height of the vertebral body in the conventional group(P>0.05).The loss rate of Cobb angle and anterior edge height of the vertebral body in the two groups was 3 days after operation and 6 days after operation.Months were lower than before operation(P<0.05).Before operation,3 days after operation,and 6 months after operation,there was no significant difference in the ODI value between the traditional Chinese medicine group and the conventional group(P>0.05).The ODI values of the patients in the two groups were lower than those before the operation 3 days after the operation and 6 months after the operation(P<0.05).Before the treatment,the 25-OH-D,N-MID andβ-CTX measured values of the traditional Chinese medicine group and the conventional group.There was no statistically significant difference between the groups(P>0.05).After treatment,the measured values of N-MID andβ-CTX in the TCM group were lower than those in the conventional group(P<0.05).Before treatment,the difference between the TCM group and the conventional group was no significant difference in the measured values of vertebral bone mineral density and femoral neck bone mineral density(P>0.05).After treatment,the measured values of vertebral bone mineral density and femoral neck bone mineral density of the TCM group were higher than those of the conventional group(P<0.05).Conclusion:The treatment of elderly osteoporotic OVCF patients with surgery combined with TCM Bushen Zhuanggu Decoction is beneficial to improve bone metabolism and relieve the condition of osteoporosis.
