摘要
目的应用实时三维超声成像诊断胎儿唇腭裂,就其诊断价值与二维超声诊断进行比较探讨。方法应用esaote Mylab 90实时三维超声显像仪,三维容积探头和二维腹部探头,对2798例16~40周胎儿唇部进行观察。结果 2798例中,应用实时三维或二维超声能看到胎儿唇部者2644例,显示率为94.5%,看不清胎儿唇部者154例。二维显示率为84.6%(2367/2798),实时三维显示率为89.7%(2510/2798)。应用实时三维或二维超声共发现胎儿唇腭裂畸形11例,与引产或足月产后相符合。二维及实时三维超声产前均检出唇腭裂10例,漏诊1例,检出率90.9%(10/11)。结论实时三维超声在诊断胎儿唇裂中图像逼真直观,对二维图像具有重要的补充作用,二者结合诊断胎儿唇腭裂是更有效的方法。
Objective Real-time three-dimensional ultrasound was used to examine normal fetal lip and abnormal cleft lip and palate,and its diagnostic value was compared with that of two-dimensional ultrasound.Methods Esaote My Lab 90 ultrasound system with threedimensional volume probe and two-dimensional abdominal probe were used to examine fetal lips in 2798 fetuses with 16-40 weeks gestational age.Results Among 2798 cases,fetal lips were detected in 2644 cases by three-dimnesional ultrasound or two-dimensional ultrasound with the detection rate of 94.5%.The fetal lips were not shown in 154 cases,The detection rate of fetal lips was 84.6%(2367/2798) by two-dimensional ultrasound and was 89.7%(2510/2798) by three-dimendional ultrasound.There were 11 cases with abnormal cleft lip and palate detected by real-time three-dimensional and two-dimensional ultrasound in this group,and all of them were confirmed by the results of abortion or postpartum.The real-time three-dimendional and two-dimensional ultrasound detected 10 cases of cleft lip and palate and missed one case with a detection rate of 90.9%(10/11).Conclusion The real-time three-dimensional ultrasound is objective and visualized in the diagnosis of fetal cleft lip and palate,which is a supplement for two-dimendionsl ultrasound.Combination of three-dimendional and two-dimensional ultrasound is a valuable method in the diagnosis of fetal cleft lip and palate.
