摘要
目的探讨胎儿正常唇腭部与唇腭裂畸形的超声检查方法、最佳检查时机及声像图特征,旨在提高产前B超对唇腭裂畸形的确诊率.方法对7 242例孕16周至产前的孕妇行常规超声检查,发现唇腭裂畸形声像予测量记录,并追踪随访,引产或足月产后对照.结果产前确诊唇腭裂畸形12例13胎,有对照的共10例11胎,其中产前诊断唇裂,产后见唇腭裂2胎,与产前B超诊断完全符合8例9胎,准确率82%(9/11).结论孕20~32周是超声检查胎儿唇腭裂畸形的最佳时机.熟练掌握以胎儿头面部横切和冠状切面为主的扫查方法,有助于快捷获取和正确识别唇腭裂声像图.唇裂时唇缘线至唇根部回声中断.牙槽突以及硬腭裂时,分别见双弧形串珠状强回声、弧形强回声带及其后马蹄形不均质中、强回声区的裂隙暗带.
Objective This study was designed to find the characteristics of normal and malformed fetal palate and the adequate time for ultrasonographic assessment, in order to promot understanding of cleft lip and palate malformation antepartum using ultrasound. Methods The routine obstetric ultrasonographic records were retrospectively reviewed on 7 242 pregnancies from 16th week of pregnancy to antepartum. The measurements and desriptions on cleff lip and palate malformation made upon discovery were compared to follow-up induced labor or termed labor. Results Twelve cases (13 fetuses ) of eleft lip and palate malformation were found antepartum and 10 cases (11 fetuses) were confirmed. Two cases of simple cleft lip antepartum were found having both cleft lip and palate malformation postpartum. Eight abnormal cases (9 fetuses) documented antepartum in ultrasonography were consistent with postpartum results, with a total accuracy of 82% (9/11). Three cases of eleft lip or palate malformation were not found. Conclusion The adequate occasion for ultrasonic imaging of fetal cleft lip and palate malformation is from 20th to 32nd weeks of gestation. Skilled scanning technique on the transverse and coronal sections of fetal head is critical for an accurate recognition of cleft lip or palate malformation.
