高强度玻璃纤维复合树脂桩核与IPS E.Max全瓷冠修复前牙缺损:80例回顾分析 被引量:7

High-strength glass fiber reinforced composite resin posts and IPS E.Max ceramic crown for repairing anterior dental defect:A retrospective analysis of 80 cases

摘要 背景:高强度玻璃纤维复合树脂桩核作为一种新型复合成品桩材料与铸造金属桩相比,具有良好的美学性能以及抗疲劳、抗腐蚀、与树脂产生强的化学结合,生物相容性好,弹性模量与牙本质相近等优点。目的:观察Parapost Fiber Lux高强度玻璃纤维复合树脂桩核系统与IPSE.Max全瓷冠修复前牙缺损的效果。设计、时间及地点:回顾性病例分析,于2007-10/2008-10在中山大学附属第三医院口腔修复科完成。对象:选择前牙缺损患者80例,男38例56颗,女42例62颗;年龄20~60岁,平均35.7岁。方法:患牙经完善的根管治疗后用高强度玻璃纤维复合树脂桩核与IPSE.Max全瓷冠进行修复。主要观察指标:以邻牙和同名牙为参照,检查牙龈有无变晦暗及龈缘黑边,牙冠颜色是否协调自然;检查桩有无松动、移位、脱落现象;全瓷冠有无脱落、变色及与邻牙的颜色协调性;修复体的边缘密合情况;摄X射线片观察根周、根尖、牙槽骨致密度影像改变,桩核与根管壁的吻合程度,有无根折、桩折等征象。结果:经过6~24个月临床随访观察,1颗桩折断,2颗全瓷冠脱落,2颗比色过程中出现偏差,剩余113颗修复体美观自然,无牙龈变色及牙龈退缩,边缘密合性好。通过X射线片检查,根管桩与根管壁黏固密合。结论:高强度玻璃纤维复合树脂桩核系统与IPSE.Max全瓷冠修复前牙缺损效果较好。 BACKGROUND: High-strength glass fiber reinforced composite resin posts have been reported compared to casting metal as excellent esthetics performance, anti-fatigue, and anti-corrosion, strong chemical constitution with resin, great biocompatibility, and similar elastic modulus to dentin. OBJECTIVE: To evaluate the therapeutic effects of Parapost Fiber Lux high-strength glass fiber reinforced composite resin posts on the restoration of the anterior dental defect and compare to IPS E.Max ceramic crown. DESIGN, TIME AND SETTING: A retrospective case analysis was performed at Department of Prosthodontics, the Third Affiliated Hospital of Sun Yat-sen University between October 2007 and October 2008. MATERIALS: A total of 80 patients with anterior dental defects, including 38 males (defected teeth =56) and 42 females (defected teeth = 62) with mean age of 35.7 years (ranged 20-60 years) were collected in this study. METHODS: After root canal therapy, the defected anterior teeth were repaired using high-strength glass fiber reinforced composite resin posts and IPS E.Max ceramic crown. MAIN OUTCOME MEASURES: As the references of adjacent teeth and homonym teeth, gum was checked whether eclipse and black gingival margin were seen, and whether color of dental crown was in coordination with natural color. In addition, loosening, displacement, and obscission were also checked. Compared to IPS E.Max ceramic crown, obscission, discoloration, and color coordination with adjacent teeth were detected. Edge closure of dental prosthetic restoration was examined'. Density changes of pedradicular, root tip, and alveolar bone were observed using X-ray as well as osculation between post and root canal wall was observed to determine whether fracture of post and root canal occurred. RESULTS: During 6-24 months clinical following up, one had post fracture, two had obscission of all-ceramic crown, two had color difference, and the rest 113 teeth were repaired perfectly, for example, either gingival discoloration or gingival recession were not observed, and edge closure was good. X-ray examination demonstrated that root canal posts were tightly adherent to root canal wall. CONCLUSION: Either high-strength glass fiber reinforced composite resin posts or IPS E.Max ceramic crown has great effects on repairing anterior dental defect.
