摘要
目的:探讨并建立可供溶栓治疗实验研究使用的肺动脉栓塞的小型猪动物模型。方法:7头实验小型猪,抽取颈内静脉血加凝血酶制备自体血栓;将导管沿上腔静脉→右心房→右心室→主肺动脉→下肺动脉,超选择至2~3级肺动脉分支,注入适量自制血栓碎块,造影确定肺栓塞发生;行实验性尿激酶溶栓治疗以确认模型。结果:7头11次栓塞后即时血管造影和随后的CT、核素灌注扫描证实栓塞部位出现肺动脉栓塞,6头次实验性尿激酶溶栓治疗显效。结论:静脉血加凝血酶是一种简单易行、效果可靠的自体血栓制备方法;经静脉途径行导管肺动脉栓塞制作肺栓塞活体实验动物模型的成功率高,可控性好;实验性尿激酶溶栓治疗确认模型有效。
Objective: To establish experimental pulmonary embolism model in the minipig for expenmental study of thrombolysis. Methods: Experiments were made on 7 minipigs. Autologous emboli were prepared from internal jugular vein blood with coagulase. A tube was inserted to pulmonary artery branch through superior vena cava→right atrium→right ventriele→main pulmonary artery→lower pulmonary artery, then the thrombus was injected into 2 - 3 grade branch of pulmonary artery. Pulmonary embolism was observed by angiography and experimental urokinase thrombolytie therapy was carried out to verify this model. Results: Eleven eases of pulmonary embolism were identified in 7 minipigs by instant angiography and subsequent CT and radionuelide perfusion, and experimental urokinase thrombolysis was effective in the treatment of 6 eases. Conclusion:Using autologous embolus is a simple and reliable method for this study. The success rate of preparing experimental model by transjugular embolization of pulmonary artery through vein is high and controllable. The minipig model established in this way is confirmed to be useful by the experimental thrombolysis treatment.
