摘要
目的:探讨缬沙坦与苯那普利对高血压患者心肌纤维化及左室肥大的逆转作用。方法:应用血管紧张素Ⅱ受体拮抗剂缬沙坦与苯那普利分别治疗高血压病伴左室肥厚患者,应用超声心动图监测治疗前后左心室各项指标,同时采用放射免疫法检测血清Ⅲ型前胶原、透明质酸、血管紧张素Ⅱ。结果:高血压病组治疗前与对照组比较,Ⅲ型前胶原,透明质酸,血管紧张素Ⅱ明显升高,一氧化氮明显下降,与治疗前比两组治疗后收缩压,舒张压,血管紧张素Ⅱ(ng/L),Ⅲ型前胶原(μg/L),透明质酸(μg/L),室间隔厚度(mm),左室后壁厚度(mm),左室舒张内径(mm),左室重量(g),左室重量指数(g/m2)均有一定的下降(P<0.05),一氧化氮(mm o l/L)有明显的升高(P<0.05)。结论:血管紧张素Ⅱ受体拮抗剂缬沙坦与苯那普利有同样的降压作用,其抑制心肌纤维化及左室肥大的机制不同,强度更强,且不良反应较少,患者容易依从,值得临床推广。
Objective To study the effect of valsartan and benazepril on left ventricle thickening and myocardial fibrosis resulting from hypertension. Methods The hypertension patients with left ventricle thickening and myocardial fibrosis were treated by valsartan and benazepril respectively. The parameters of left ventricle and PC Ⅲ , HA, and Ang Ⅱ were measured respectively. Results PC Ⅲ, HA and Ang Ⅱin hypertension group were significantly higher than those in control group, and NO in hypertension group was significantly lower than that in control group before treatment. SBP, DSP, Ang Ⅱ , PC Ⅲ, HA, IVST, LVPW, LVDd, LVM, and LVMI in both groups after treatment were significantly lower than those before treatment(P〈0. 05), and NO was significantly higher than that before treatment (P〈0. 05). Conclusion Valsartan is as the same as benazepril to lower hypertension, inhibit left ventricle thickening and myocardial fibrosis. Valsartan has less adverse effect than benazepril.
