足底驱动型下肢康复机器人结合PNF对脑卒中偏瘫患者下肢功能的影响研究

The Effect of Plantar-Driven Lower Limb Rehabilitation Robot Combined with PNF on the Lower Limb Function of Stroke Patients with Hemiplegia

摘要 目的研究脑卒中偏瘫患者采用足底驱动型下肢康复机器人结合本体感觉神经肌肉促进技术(proprioceptive neuromuscular facilttation,PNF)对其下肢功能的影响.方法选取2020年1—11月该院收治的98例脑卒中偏瘫患者,使用随机数字表法分为两组,各49例,采用常规康复训练的一组为对照组,采用足底驱动型下肢康复机器人结合PNF的一组为研究组,对比两组干预后的临床效果.结果研究组满意度95.92%高于对照组的73.47%,差异有统计学意义(P<0.05).干预前,两组的下肢Fugl-Meyer运动功能评定(Fugl Meyer assessment,FMA)、Berg平衡量表(Berg balance scale,BBS)、10 m最大步行速度(maximum walking speed,MWS)、生活质量量表(shore form 36 questionnaive SF-36)评分对比,差异无统计学意义(P>0.05);干预后,研究组的下肢FMA评分、BBS评分、10 m MWS、社会功能评分、一般健康状况评分分别为(26.35±3.41)分、(43.62±9.28)分、(5.82±1.01)m/min、(69.38±11.27)分、(68.97±8.35)分,均高于对照组的(20.81±3.54)分、(31.28±7.48)分、(4.01±0.71)m/min、(54.82±10.32)分、(61.24±8.05)分,组间差异有统计学意义(P<0.05).结论脑卒中偏瘫患者采用足底驱动型下肢康复机器人结合PNF,可显著改善其下肢运动功能,加快患者恢复速度,提升其生活质量及满意度,值得临床借鉴. Objective To study the effects of plantar-driven lower extremity rehabilitation robots combined with proprioceptive neuromuscular facilitation(PNF)in stroke patients with hemiplegia on their lower extremity functions.Methods A total of 98 stroke patients with hemiplegia admitted to the hospital from January to November 2020 were selected and divided into two groups using a random number table method,with 49 cases in ecch group.The group took regular rehabilitation training was set as the control group,and the group took the plantar-driven lower limb rehabilitation robot combined with PNF was set as the study group.The clinical effects of the two groups after intervention were compared.Results The satisfaction rate of the study group was 95.92%,which was higher than the control group's 73.47%,the difference was statistically significant(P<0.05).Before the intervention,there were no statistically significant difference between the two groups of lower limb Fugl-Meyer assessment(FMA),Berg balance scale(BBS),10 m maximum walking speed(MWS),and short form 36 questionnaire(SF-36)scores(P>0.05);after the intervention,the lower limb FMA score,BBS score,10 m MWS,social function score,and general health status score of the study group were(26.35±3.41)points,(43.62±9.28)points,(5.82±1.01)m/min,(69.38±11.27)points,(68.97±8.35)points,which were higher than the control group's(20.81±3.54)points,(31.28±7.48)points,(4.01±0.71)m/min,(54.82±10.32)points,(61.24±8.05)points,the differences between the groups were statistically significant(P<0.05).Conclusion The use of plantar-driven lower extremity rehabilitation robots combined with PNF for stroke patients with hemiplegia can significantly improve their lower extremity motor function,accelerate their recovery,improve their quality of life,and increase their satisfaction,which is worthy of clinical reference.
