重复性训练对卒中患者偏瘫上肢痉挛改善的疗效观察 被引量:27

摘要 目的研究重复性训练对卒中患者偏瘫上肢痉挛改善的情况。方法选择卒中偏瘫患者31例。分为康复组(13例)和对照组(18例)。采用运动器械MOTOmedviva2进行上肢重复性训练3周,以两个手摇板记录每侧肢体肌张力变化情况、脚板力矩。并以肌电图检测患侧康复前、后屈伸肌群收缩变化情况。采用卒中损伤评定量表(SIAS)、徒手肌力检测(MMT)及改良痉挛评定量表(Asworth)和关节活动度评定。结果训练前康复组SIAS、MMT及Asworth评分各为1.28±0.25、1.42±0.35及3.42±0.68,对照组为1.18±0.04、1.24±0.31及3.21±0.64;训练后评分各为3.85±0.55、3.28±0.66及0.86±0.15,2.63±0.53、2.49±0.62及1.21±0.24。训练后两组比较,P<0.01。关节活动度评定示康复组患者肩关节和肘关节活动度增加。结论患侧上肢在经过重复性运动训练后,力量增加,关节活动度扩大,肌痉挛降低。 Objective To study repetitive training in the improvement of hemiplegic upper extremity spasticity in patients with stroke. Methods Thirty-one hemiplegic patients with stroke were assigned to rehabilitation group (n = 13 ) and control group (n = 18). Exerciser MOTOmed viva 2 was used for repetitive training on the upper-extremity for 3 weeks. The changes of muscular tension on each side of limb, feet torque and position signals on an ergometer were recorded. The changes of muscle contraction on the affected sides were measured by electromyogram before and after rehabilitation. SIAS, manual muscle test (MMT), Ashworth scale, and range of motion evaluations were used for assessment. Results The scores of SIAS, MMT, and Ashworth scale before the training in the rehabilitation and control groups were 1.28 ± 0. 25 and 1.42 ± 0. 35, 3.42 ± 0.68, 1.18 ± 0. 04 and 1.24 ± 0. 31, and 3.21 ± 0. 64, respectively. Their scores after the training were 3.85 ± 0. 55 and 3.28 ± 0.66, 0. 86 ± 0.15, 2.63 ± 0.53 and 2. 49 ± 0. 62, and 1.21 ± 0. 24, respectively. The comparison between both groups was P 〈 0. 01 after training. The increased range of motion of shoulder and elbow were measured using the range-of-motion assessment. Conclusion The strength of the affected upper extremities increased, joint range of motion increased, and muscle spasm improved after the repetitive movement training.
