摘要
目的探讨快速康复外科护理在手足外科游离皮瓣移植术患者中的应用效果.方法选择该院手足外科2019年12月—2020年11月收治的行游离皮瓣移植术患者78例,按照随机数字表法分为对照组(n=39,采用常规围术期护理)与观察组(n=39,采用快速康复外科护理),比较两组的术后康复相关指标、护理满意度,并采集患者术前、术后空腹静脉血检测皮质醇水平,分析患者应激反应情况.结果观察组术后使用抗生素时间、皮瓣愈合时间、住院时间、疼痛评分分别为(6.04±0.85)d、(15.33±4.62)d、(16.16±4.86)d、(1.53±0.59)分,均优于对照组,组间差异有统计学意义(P<0.05).观察组术后的皮质醇水平为(395.22±28.43)nmol/L,低于对照组,差异有统计学意义(P<0.05).观察组的护理满意度为94.87%,高于对照组的76.92%,差异有统计学意义(P<0.05).结论手足外科游离皮瓣移植术患者采用快速康复外科能够有效减轻应激反应,改善患者的护理服务体验,有利于患者的快速康复,值得临床推广.
Objective To explore the application effect of fast track surgery nursing in patients undergoing free flap transplantation in hand and foot surgery.Methods A total of 78 patients undergoing free flap transplantation who were admitted to the hospital from December 2019 to Movember 2020 in the hand and foot surgery department were selected and divided into a control group(n=39,using routine perioperative nursing)and an observation group(n=39,using fast track surgery nursing).The postoperative rehabilitation related indicators and nursing satisfaction were compared between the two groups,the fasting venous blood of the patients before and after the operation was collected to detect the cortisol level,and the stress response of the patients was analyzed.Results The postoperative antibiotic use time,flap healing time,hospitalization time,and pain score in the observation group were(6.04±0.85)d,(15.33±4.62)d,(16.16±4.86)d,and(1.53±0.59)points,respectively,which were better than those of the control group,and the differences between the groups were statistically significant(P<0.05).The postoperative cortisol level in the observation group was(395.22±28.43)nmol/L,which was lower than that of the control group,and the difference was statistically significant(P<0.05).The nursing satisfaction of the observation group was 94.87%,which was higher than 76.92%of the control group,and the difference was statistically significant(P<0.05).Conclusion Using fast track surgery nursing for patients undergoing hand and foot surgery free flap transplantation can effectively reduce the stress response and improve the patient’s nursing service experience,which is conducive to the rapid recovery of the patient and is worthy of clinical promotion.
