摘要
目的探究子宫内膜息肉不孕患者采用宫腔镜电切术治疗对其围术期指标、并发症及妊娠情况的影响.方法纳入我院2018年1月-2019年11月收治的88例子宫内膜息肉不孕患者,采用随机数字表法分为对照组44例和研究组44例.对照组采用宫腔镜刮宫术治疗,研究组采用宫腔镜电切术治疗,随访至术后1年.对比两组围术期指标、手术前后月经量变化、并发症发生率、复发率及妊娠情况.结果研究组手术时间(10.78±2.05)min、术中出血量(4.95±0.69)mL、住院时间(2.96±0.38)d与对照组的(10.99±1.87)min、(5.03±0.66)mL、(3.10±0.34)d比较,组间差异无统计学意义(P>0.05).术后3个月,研究组的月经量为(175.85±64.29)mL,少于对照组的(243.15±69.82)mL,差异有统计学意义(P<0.05).研究组并发症发生率为2.27%,低于对照组的11.36%,差异无统计学意义(P>0.05).研究组妊娠成功率为70.45%,高于对照组的45.45%,复发率及流产率分别为2.27%、6.82%,均低于对照组的18.18%、25.00%,组间差异有统计学意义(P<0.05).结论宫腔镜电切术在子宫内膜息肉不孕患者治疗中的临床应用效果确切,能够有效促进患者恢复月经量,提升妊娠成功率,降低术后并发症发生率、复发率及流产率,值得推广.
Objective To explore the effect of hysteroscopic electroresection on perioperative indicators,complications and pregnaney in infertile patients with endometrial polyps.Methods A total of 88 infertile patients with endometrial polyps admitted to the hospital from January 2018 to November 2019 were included and divided into a control group of 44 cases and a study group of 44 cases by random number table method.The control group was treated with hysteroscopic curettage,and the study group was treated with hysteroscopic electroresection.The patients were followed up for I year after operation.The perioperative indicators,changes in menstrual flow before and after surgery,complication rates,recurrence rates and pregnancy were compared between the two groups.Results The operation time(10.78+2.05)min,intraoperative blood loss(4.95+0.69)mL,hospital stay(2.96±0.38)d in the study group were compared with(10.99±1.87)min,(5.03±0.66)mL,(3.10±0.34)d of the control group,and the differences between the groups were not statistically significant(P>0.05).3 months after operation,the menstrual flow in the study group was(175.85±64.29)mL,which was less than(243.15±69.82)mL in the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the study group was 2.27%,which was lower than 11.36%in the control group,and the difference was not statistically significant(P>0.05).The pregnancy success rate in the study group was 70.45%,which was higher than 45.45%in the control group,the recurrence rate and miscarriage rate were 2.27%and 6.82%,respectively,which were both lower than 18.18%and 25.00%in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion The clinical application effect of hysteroscopic electroresection in the treatment of infertile patients with endometrial polyps is accurate,which can effectively promote the recovery of menstrual flow,improve the success rate of pregnancy,and reduce the incidence of postoperative complications,recurrence and miscarriage.it is worth promoting.
