摘要
目的探讨中风后偏瘫患者在中频脉冲电治疗基础上采用雷火灸联合循经推拿治疗的效果.方法选取2020年8月—2021年8月在宜宾市第二中医医院就诊的60例中风后偏瘫患者,采用随机数字表法将其分为对照组与研究组.对照组(n=30)仅采用中频脉冲电治疗,研究组(n=30)在对照组基础上采用雷火灸联合循经推拿治疗.比较两组治疗前后的日常生活活动能力(ADL)评分、Fugl-Meyer运动功能评定量表(FMA)评分、美国国立卫生研究院卒中量表(NIHSS)评分、改良Ashworth痉挛分级及治疗期间的安全性.结果治疗后,两组的ADL评分、FMA评分、NIHSS评分、Ashworth分级均较治疗前改善,且研究组ADL评分、FMA评分高于对照组,NIHSS评分低于对照组,腕屈肌、踝屈肌Ashworth分级优于对照组,组间差异有统计学意义(P<0.05).治疗期间,两组均未出现不良反应.结论中风后偏瘫患者在中频脉冲电治疗基础上采用雷火灸联合循经推拿治疗,可提高肢体功能及日常活动能力,减轻神经缺损程度,安全有效,值得临床推广使用.
Objective To investigate the effect of thunder-fire moxibustion combined with meridian massage on patients with hemiplegia after stroke based on medium frequency pulsed electric therapy.Methods A total of 60 patients with poststroke hemiplegia treated in Yibin Second Hospital of Traditional Chinese Medicine from August 2020 to August 2021 were selected and randomly divided into a control group and a study group by random number table method.The control group(n=30)was only treated with medium frequency pulsed electric therapy,while the research group(n=30)was treated with thunder-fire moxibustion combined with meridian massage on this basis.The activities of daily living(ADL)score,Fugl-Meyer assessment(FMA)score,National Institutes of Health stroke scale(NIHSS)score,modified Ashworth spasm scale between the two groups before and after treatment and the safety during treatment were compared.Results After treatment,the ADL scores,FMA scores,NIHSS scores and Ashworth degrees of the two groups were improved compared with before treatment,and the ADL score and FMA score of the study group were higher than those of the control group,NIHSS score was lower than that of the control group,and Ashworth degrees of flexor carpi and ankle muscles were better than those of the control group,the differences between the groups were statistically significant(P<0.05).There were no adverse reactions in the two groups during treatment.Conclusion On the basis of medium frequency pulsed electric therapy,thunder-fire moxibustion combined with meridian massage therapy for patients with hemiplegia after stroke can improve limb function and daily activity ability,reduce the degree of nerve defect,which is safe and effective and worthy of clinical application.
