摘要
目的探讨电针阴部神经刺激疗法联合盆底肌康复训练在良性前列腺增生电切术后尿失禁患者中的应用效果.方法选择我院2020年1月—2021年8月收治的78例良性前列腺增生电切术后尿失禁患者为研究对象,随机分为对照组和试验组,各39例.对照组采用盆底肌康复训练,试验组采用电针阴部神经刺激疗法联合盆底肌康复训练.观察并比较两组临床疗效、治疗前后1 h漏尿量、24 h漏尿次数、生活质量及尿失禁严重程度.结果试验组患者治疗总有效率高于对照组,差异有统计学意义(P<0.05).治疗后,两组患者1 h漏尿量、24 h漏尿次数均较治疗前减少,且试验组1 h漏尿量、24 h漏尿次数均少于对照组,组间差异有统计学意义(P<0.05).治疗后,两组患者尿失禁生活质量问卷(I-QOL)评分较治疗前升高,国际尿失禁咨询委员会尿失禁问卷简表(ICI-Q-SF)评分均较治疗前降低,且试验组I-QOL评分高于对照组,ICI-Q-SF评分低于对照组,组间差异有统计学意义(P<0.05).结论电针阴部神经刺激疗法联合盆底肌康复训练治疗良性前列腺增生电切术后尿失禁患者的疗效显著,可减少1 h漏尿量和24 h漏尿次数,明显改善患者的生活质量.
Objective To investigate the application effect of electroacupuncture pudendal nerve stimulation therapy combined with pelvic floor muscle rehabilitation training in patients with urinary incontinence after electroresection of benign prostatic hyperplasia.Methods 78 patients with urinary incontinence after electroresection of benign prostatic hyperplasia who were treated in the hospital from January 2020 to August 2021 were selected as the research objects,and they were randomly divided into a control group and an experimental group,with 39 cases in each group.The control group received pelvic floor muscle rehabilitation training,while the experimental group received electroacupuncture pudendal nerve stimulation therapy combined with pelvic floor muscle rehabilitation training.The clinical efficacy,1-hour urine leakage volume,24-hour urine leakage frequency,quality of life and the severity of urinary incontinence before and after treatment were observed and compared between the two groups.Results The total effective rate of patients in the experimental group was higher than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the 1-hour urine leakage and the 24-hour urine leakage frequency in the two groups were reduced compared with those before treatment,and the 1-hour urine leakage and the 24-hour urine leakage frequency in the experimental group were less than those in the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the scores of Incontinence quality of life questionnaire(I-QOL)in the two groups were increased,and the scores of International Consultation on Incontinence Questionnaire short form(ICI-Q-SF)were decreased compared with those before treatment,and the I-QOL score of the experimental group was higher than that of the control group,and the ICI-Q-SF score of the experimental group was lower than that of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Electroacupuncture pudendal nerve stimulation therapy combined with pelvic floor muscle rehabilitation training has significant curative effect on urinary incontinence patients after electroresection of benign prostatic hyperplasia,can reduce the amount of urine leakage in 1 hour and the frequency of urine leakage in 24 hours,and significantly improve the quality of life of patients.
