摘要
目的探讨空气肢体压力治疗仪联合早期运动疗法对脑出血后下肢深静脉血栓(DVT)的预防效果。方法选取阳信县人民医院2020年1月—2022年12月收治的88例脑出血患者为研究对象,采用随机数字表法将其分为对照组与观察组,各44例。对照组采用早期运动疗法治疗,观察组在对照组基础上联用空气肢体压力治疗仪治疗,比较两组治疗前后的下肢血液循环、肢体功能、日常活动能力和预后恢复情况。结果治疗后7 d,观察组活化部分凝血酶时间、凝血酶时间和凝血酶原时间分别为(32.21±3.74)s、(18.22±1.56)s、(17.01±1.10)s,均长于对照组的(27.82±2.50)s、(13.66±1.17)s、(14.31±1.05)s,组间差异有统计学意义(P<0.05)。治疗后7 d,观察组住院时间为(12.16±0.79)d,短于对照组的(16.24±1.05)d,差异有统计学意义(P<0.05)。治疗前7d,两组股静脉与腘静脉血流速度比较,差异无统计学意义(P>0.05)。治疗后7 d,观察组股静脉与腘静脉血流速度分别为(33.21±4.74)cm/s、(18.22±3.36)cm/s,均快于对照组的(29.82±3.50)cm/s、(16.30±2.59)cm/s,组间差异有统计学意义(P<0.05)。观察组术后1个月内下肢DVT发生率4.55%低于对照组的20.45%,差异有统计学意义(P<0.05)。治疗后7 d,观察组日常生活能力评定量表评分为(91.79±1.32)分,高于对照组的(87.07±3.21)分,差异有统计学意义(P<0.05)。治疗后7 d,观察组日常活动能力量表评分为(88.34±5.19)分,高于对照组的(76.21±3.09)分,差异有统计学意义(P<0.05)。治疗后7 d,观察组肢体运动功能量表评分为(85.83±2.02)分,高于对照组的(78.00±1.19)分,差异有统计学意义(P<0.05)。结论空气肢体压力治疗仪联合早期运动疗法能够有效改善患者下肢血液循环,预防脑出血DVT形成。
Objective To investigate the preventive effect of the air pressure wave therapy device combined with early mobilization therapy in preventing deep venous thrombosis(DVT)in the lower limbs of patients with cerebral hemorrhage.Methods 88 patients with cerebral hemorrhage admitted to Yangxin County People's Hospital from January 2020 to December 2022 were selected as the study objects and randomly divided into a control group and an observation group,with 47 cases in each group.The control group was treated with early mobilization therapy and the observation group was treated with the air pressure wave therapy device in addition to the control group.The study compared the pre-and post-treatment lower limb blood circulation,limb function,daily activity,and recovery outcomes between the two groups.Results 7 days after treatment,the activated partial thrombin time,thrombin time and prothrombin time in the observation group were(32.21±3.74)s,(18.22±1.56)s and(17.01±1.10)s,respectively,which were longer than(27.82±2.50)s,(13.66±1.17)s and(14.31±1.05)s of the control group,and the differences were statistically significant(P<0.05).7 days after treatment,the hospitalization time of observation group was(12.16±0.79)d,which was shorter than(16.24±1.05)d of the control group,and the difference was statistically significant(P<0.05).7 days before treatment,the femoral vein and popliteal vein blood flow velocity between the two groups showed no statistical significance(P>0.05).7 days after treatment,the femoral vein and popliteal vein blood flow velocity in the observation group was(33.21±4.74)cm/s and(18.22±3.36)cm/s,respectively,both faster than(29.82±3.50)cm/s and(16.30±2.59)cm/s,the differences between groups were statistically significant(P<0.05).The incidence of DVT in the observation group was 4.55%lower than 20.45%in the control group,and the difference was statistically significant(P<0.05).7 days after treatment,the daily living ability rating scale score of the observation group was(91.79±1.32)points,which was higher than(87.07±3.21)points of the control group,and the difference was statistically significant(P<0.05).7 days after treatment,the daily activity ability scale score of the observation group was(88.34±5.19)points,which was higher than that of the control group(76.21±3.09)points,and the difference was statistically significant(P<0.05).7 days after treatment,the motor function scale score of the observation group was(85.83±2.02)points,which was higher than(78.00±1.19)points of the control group,and the difference was statistically significant(P<0.05).Conclusion The air pressure wave therapy device combined with early mobilization therapy effectively improves lower limb blood circulation,prevents DVT formation in patients with cerebral hemorrhage.
