摘要
目的探讨虚拟现实技术(VR)训练辅助低频重复经颅磁刺激(rTMS)对重度颅脑外伤术后患者功能恢复的影响。方法选择2021年8月—2023年3月我院收治的98例重度颅脑外伤术后患者为研究对象,按随机数字表法将其分为对照组和观察组,每组49例。对照组予以常规康复训练,观察组在常规康复训练的基础上采用VR训练辅助rTMS干预。比较两组P300水平、认知功能、运动功能、平衡功能和日常生活能力。结果干预后,观察组P300潜伏期较对照组短,P300振幅和蒙特利尔认知评估量表评分较对照组高,组间差异有统计学意义(P<0.05)。干预后,观察组Fugl-Meyer运动功能评定量表-上肢功能部分、Fugl-Meyer运动功能评定量表-下肢功能部分、Berg平衡量表、巴氏量表评分均高于对照组,组间差异有统计学意义(P<0.05)。结论在重度颅脑外伤术后患者采用VR训练辅助rTMS干预,可抑制大脑皮质兴奋调节P300水平定位,改善患者认知、平衡和运动功能,提高日常生活能力。
Objective To explore the effect of virtual reality technology(VR)training assisted low-frequency repetitive transcranial magnetic stimulation(rTMS)on functional recovery in patients with severe traumatic brain injury after surgery.Methods 98 postoperative patients with severe traumatic brain injury admitted to our hospital from August 2021 to March 2023 were selected as the study objects and randomly divided into a control group and an observation group,with 49 patients in each group.The control group received routine rehabilitation training,while the observation group received VR training assisted with rTMS intervention on the basis of routine rehabilitation training.Compare the P300 levels,cognitive function,motor function,balance function,and daily living ability between two groups.Results After the intervention,the latency of P300 in the observation group was significantly shortened compared to the control group,and the amplitude of P300 and MoCA score were increased compared to the control group,with statistical significance between the two groups(P<0.05).After intervention,the scores of FMA-UE,FMA-LE,BBS,and BI in the observation group were significantly higher than those in the control group,with statistical significance between the two groups(P<0.05).Conclusion VR training combined with rTMS intervention nursing can inhibit cortical excitatory regulation of P300 level localization,improve cognitive,balance,and motor function,and enhance daily living ability in patients with severe traumatic brain injury after surgery.
